The service coordinator is part of a robust and highly collaborative team. The service coordinator … Protects and promotes the health, safety, dignity, and quality of life for individuals served within our Developmental Services program. Develops and implements person-centered plans in accordance with federal and state requirements. Monitors service delivery and its effectiveness. Conducts monthly home visits to assess participants' wellbeing and to monitor their homes for continued safety and suitability. Communicates effectively and ensures that all team players collaborate in service to the individual's needs
