We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes--not rushed visits and volume-driven metrics. In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital. Required Qualifications:

Graduate of an accredited NP or PA program Current unrestricted APRN or PA license National board certification BLS certification

Preferred Qualifications:

Primary care or outpatient experience Chronic disease management experience Interest in value-based care and population health
