We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes--not rushed visits and volume-driven metrics.
In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.

Compensation for this role includes: Competitive base salary aligned with experience, scope of practice, and panel responsibility Value-based incentive opportunities tied to quality measures, preventive care, chronic disease management, and patient engagement Incentives designed to reward thoughtful, proactive care rather than RVU production For experienced APPs, incentives reflect independent panel management, longitudinal patient outcomes, and contribution to population health goals.
For newer or early-career APPs, the compensation model emphasizes stability, structured onboarding, mentorship, and a gradual ramp-up of panel and incentive expectations.

Required Qualifications:
Graduate of an accredited NP or PA program Current unrestricted APRN or PA license National board certification BLS certificationPreferred Qualifications:
