Part-Time Job Summary:

Roots Community Home Care LLC is seeking a dedicated and experienced Registered Nurse (RN) Coordinator to oversee clinical services for adults in licensed foster care settings. This position ensures compliance with North Dakota Department of Health and Human Services (ND HHS) regulations while promoting high-quality, person-centered care. The RN Coordinator supports individuals with aging-related conditions, dementia, and traumatic brain injury (TBI) and plays a key role in care planning, medication oversight, and staff training.

Key Responsibilities:

Conduct comprehensive nursing assessments and ongoing reassessments per ND HHS guidelines Develop, implement, and monitor Individual Program Plans (IPP) and person-centered care plans in compliance with ND Medicaid Waiver and HCBS standards Ensure all services meet North Dakota Administrative Code (NDAC) requirements for adult foster care and home/community-based services Oversee and maintain accurate Medication Administration Records (MARs) in accordance with state medication administration regulations Provide clinical oversight to foster care providers and direct support staff Train staff on: Medication administration Infection control Client-specific needs (dementia, TBI, behavioral supports) Utilize THERAP or similar electronic documentation systems for: Service documentation Incident reporting Health tracking Participate in care conferences, IPP meetings, and interdisciplinary team coordination Ensure compliance with HIPAA, abuse/neglect reporting laws, and ND vulnerable adult protections Assist with state audits, licensing reviews, and surveys Monitor client health status and respond to changes in condition Required Qualifications (ND-Specific): Active, unrestricted Registered Nurse (RN) license in North Dakota (ND Board of Nursing compliant) Graduate of an accredited ASN or BSN nursing program Current CPR/BLS certification

Experience Requirements:

Minimum 2-5 years of nursing experience , preferably in: Adult foster care, group homes, assisted living, or long-term care Home health or HCBS (Home and Community-Based Services) settings Direct experience supporting: Older adults / geriatric population Individuals with dementia or Alzheimer's disease Individuals with Traumatic Brain Injury (TBI) or cognitive impairments

Regulatory & Compliance Knowledge:

Working knowledge of: North Dakota Administrative Code (NDAC) related to adult foster care and HCBS ND Medicaid Waiver programs and service delivery standards ND Department of Health and Human Services (HHS) licensing requirements Experience ensuring compliance with: Medication administration standards Documentation and service delivery requirements Ability to support agency readiness for: State surveys, audits, and licensing reviews

Clinical & Technical Skills:

Strong experience with: Care planning and Individual Program Plans (IPP) Medication Administration Record (MAR) creation and auditing Nursing assessments and clinical oversight Familiarity with THERAP or similar EHR systems (preferred) Knowledge of chronic disease management, dementia care best practices, and behavioral supports

Leadership & Training:

Proven ability to: Train and educate direct support staff and foster providers Conduct staff competency evaluations Provide ongoing clinical supervision and support Strong leadership and team collaboration skills

Additional Requirements:

Valid driver's license and reliable transportation (required for travel between homes) Ability to pass ND background checks and abuse registry screening Strong organizational, documentation, and time-management skills Ability to work independently and respond to urgent clinical needs

Preferred Qualifications:

Bachelor of Science in Nursing ( BSN ) Experience working with ND Medicaid Waiver or HCBS programs Prior experience with state licensing surveys or audits Experience in staff development or nurse education roles
