Memory Care Supervisor (LPN Highly Preferred) Are you an LPN with a passion for Memory Care and developing strong care teams? We are seeking a compassionate, confident Memory Care Supervisor (Cottage Care Director) to lead the daily operations, resident care, and team within our dedicated Memory Care neighborhood. This is an excellent opportunity for an LPN with Assisted Living and Memory Care experience who is ready to combine clinical knowledge with hands-on leadership. You will work closely with the Resident Care Director while taking ownership of the day-to-day resident experience, associate development, assessments, care coordination, and overall success of the Memory Care neighborhood. LPN licensure is highly preferred. Candidates who are not licensed nurses must hold a current Certified Med Tech certification. What You'll Do Lead and supervise Med Techs, CNAs, Resident Assistants, and other Memory Care associates. Recruit, hire, orient, train, coach, and develop the Memory Care team. Promote accountability, teamwork, strong communication, and compassionate resident care. Complete and support resident assessments and identify changes in condition or care needs. Develop and implement individualized Plans of Care. Monitor physician orders and ensure appropriate follow-through. Oversee medication administration processes and support medication compliance. Review resident falls and other clinical concerns to identify appropriate interventions. Partner closely with the Resident Care Director to ensure strong clinical oversight and continuity of care. Coordinate with families, medical providers, and other community department leaders regarding resident needs. Participate in resident and family care plan meetings. Coach and educate associates on dementia care, resident interaction, medication practices, and service expectations. Support person-centered activities and life-skills programming for residents living with cognitive impairment. Participate as an active member of the community management team. Support compliance with applicable state regulations, company policies, and HIPAA requirements. Perform additional responsibilities as outlined in the full job description. What We're Looking For Our ideal candidate is an LPN who understands both the clinical and human side of Memory Care . You should be comfortable assessing resident needs, recognizing changes in condition, communicating with families, supporting medication management, and using sound clinical judgment. Just as importantly, you should be able to lead people. We are looking for someone who can teach, coach, develop, and hold a team accountable while building a strong partnership with the Resident Care Director and broader community leadership team. Qualifications Current LPN license in good standing is highly preferred. Candidates without an LPN license must hold a current Certified Med Tech certification . Previous Memory Care and dementia experience required. Previous supervisory or leadership experience strongly preferred. Experience in an Assisted Living environment preferred. Knowledge of medication administration practices and procedures. Experience with resident assessments and individualized care planning preferred. Strong clinical observation, critical-thinking, and problem-solving skills. Ability to recognize and appropriately communicate changes in resident condition. Strong leadership skills with the ability to train, coach, and develop direct-care associates. Excellent communication skills with residents, families, associates, and medical providers. Understanding of the social, psychological, physical, and recreational needs of residents living with cognitive impairment. Certified Dementia Practitioner (CDP) certification is a plus. Must be at least 21 years of age. Must possess and maintain a valid driver's license and acceptable Motor Vehicle Record in accordance with Company Fleet Safety standards.
