Program Director / Program Manager About Sofiya's Village THP Sofiya's Village Transitional Housing Program (THP) provides a safe, structured, and supportive home environment for AB 12-eligible foster youth who are transitioning toward independence and adulthood. Our program supports young adults as they develop the life skills, stability, and resources necessary to successfully transition to permanent housing and self-sufficiency. We are seeking a compassionate, dependable, and organized Part-Time Site Supervisor to provide direct supervision, support, and case management services to residents living in the THP home. Position Summary The Part-Time Site Supervisor is responsible for the daily supervision and support of AB 12 residents living in the THP home. The Site Supervisor will maintain a safe and structured living environment while providing case management services designed to help residents achieve their individual goals and successfully transition to independent living. The Site Supervisor will work closely with residents, program staff, county social workers, probation officers, and other service providers to coordinate services, monitor resident progress, address concerns, and ensure that residents receive appropriate support. The ideal candidate is compassionate and youth-centered while also being comfortable establishing boundaries, enforcing program expectations, documenting services, and holding residents accountable. Essential Duties and ResponsibilitiesResident Supervision Provide direct supervision, guidance, and support to AB 12 residents residing in the THP home. Maintain a safe, stable, structured, and respectful residential environment. Monitor resident activities and ensure compliance with THP policies, house rules, curfews, and program expectations. Conduct regular check-ins with residents regarding their well-being, responsibilities, and progress. Encourage residents to develop independence, accountability, healthy routines, and essential life skills. Address conflicts, behavioral concerns, and violations of program expectations using trauma-informed and supportive approaches. Maintain appropriate professional boundaries and confidentiality. Respond appropriately to emergencies, safety concerns, and other situations requiring immediate attention. Case Management Services Provide individualized case management services to assigned AB 12 residents. Meet regularly with residents to identify strengths, needs, goals, and barriers to successful independence. Develop, implement, and monitor individualized service and transition plans. Assist residents in establishing and achieving goals related to housing, employment, education, finances, health, transportation, and independent living. Connect residents with appropriate community resources and supportive services. Provide referrals and follow up with service providers to ensure continuity of care and support. Assist residents in accessing benefits, healthcare, behavioral health services, educational programs, employment opportunities, and other resources. Help residents develop practical independent living skills, including budgeting, meal planning, household responsibilities, communication, problem-solving, and time management. Support residents in preparing for successful transition from THP to permanent housing and greater self-sufficiency. Collaboration with County and Community Partners Work collaboratively with the resident's county social worker , probation officer when applicable, and other members of the resident's support team. Maintain regular and professional communication with county social workers regarding resident progress, needs, services, and concerns. Coordinate with probation officers regarding applicable resident requirements, appointments, conditions, and progress when applicable. Participate in case conferences, multidisciplinary team meetings, court-related meetings, and other meetings as appropriate. Communicate significant incidents, safety concerns, behavioral issues, or changes in resident circumstances to the appropriate county or program personnel in accordance with program policies and confidentiality requirements. Coordinate referrals and services with county agencies, community-based organizations, educational institutions, healthcare providers, employers, and other service partners. Advocate for residents while maintaining appropriate professional boundaries and supporting compliance with applicable program and supervision requirements. Documentation & Reporting Maintain accurate and timely case notes, progress notes, resident records, and other required documentation. Document resident contacts, services provided, referrals, appointments, incidents, and progress toward individualized goals. Complete incident reports and other required program documentation. Maintain accurate records of resident participation and compliance with program requirements. Communicate significant concerns and changes in resident status to the Program Manager or Program Director. Maintain confidentiality and protect sensitive resident information in accordance with organizational policies and applicable laws and regulations. Accountability & Program Compliance Clearly communicate THP expectations, policies, and resident responsibilities. Hold residents accountable in a fair, consistent, respectful, and developmentally appropriate manner. Assist residents in understanding and addressing barriers to successful participation in the program. Support residents in meeting applicable case plan, probation, county, educational, employment, and other identified requirements. Use trauma-informed care, conflict resolution, and de-escalation techniques when addressing challenging situations. Promote positive relationships and a supportive community within the home. Qualifications High school diploma or GED required; college degree or coursework in social work, human services, psychology, behavioral health, sociology, or a related field preferred. Experience working with foster youth, transition-age youth, young adults, or individuals experiencing housing instability preferred. Experience providing case management, residential services, supportive services, or youth services preferred. Experience working with county social services, probation, child welfare, or other community-based agencies is highly desirable. Strong communication, organization, documentation, and interpersonal skills. Ability to establish appropriate professional boundaries while developing positive relationships with young adults. Ability to balance compassion, mentorship, accountability, and enforcement of program expectations. Ability to work collaboratively with social workers, probation officers, service providers, and other professionals. Ability to maintain confidentiality and exercise sound professional judgment. Ability to remain calm and professional during challenging or crisis situations. Basic computer skills and ability to complete electronic documentation. Must be dependable, punctual, and reliable. Must successfully complete required background screening and any other employment requirements. Preferred Skills and Experience Knowledge of California's Extended Foster Care/AB 12 program. Experience working with Transitional Housing Placement programs. Experience working with county child welfare or social service agencies. Experience collaborating with probation officers or juvenile justice professionals. Knowledge of resources available to transition-age foster youth. Experience with trauma-informed care and de-escalation. Experience supporting youth with independent living and self-sufficiency skills. CPR/First Aid certification or willingness to obtain. Bilingual or multilingual skills are a plus. Ideal Candidate The ideal candidate is compassionate, dependable, organized, and committed to helping young adults succeed . The Site Supervisor should be comfortable serving as both a supportive mentor and an accountable program representative. They must be able to establish trusting professional relationships with AB 12 residents while maintaining clear boundaries and program expectations. This individual should also be comfortable communicating and collaborating with county social workers, probation officers, and other professionals involved in the resident's care, supervision, and transition to independence. Work Environment & Schedule This is a part-time, onsite position within the Sofiya's Village THP home. The Site Supervisor works directly with residents and should be comfortable working in a residential environment where resident needs and situations may change throughout the shift.
