Community Support Services Program Supervisor 3.4 3.4 out of 5 stars Hammond, LA 70401 Full-time TARC 51 reviews Full-time Job Overview We are seeking a dedicated Community Support Services Program Supervisor to lead and oversee community-support programs serving individuals with developmental disabilities and coordinate all aspects of services provided under the Community Support Services Program. Duties Responsible for the development and implementation of the provider plan of care Responsible for the monitoring of services, data collection and report compilation. Responsible for visiting each assigned family monthly and completing a monthly SIL log work and report. Responsible for visiting each assigned family quarterly with a Support Coordinator Responsible for maintaining compliance with SIL requirements. Responsible for maintaining compliance with Social Security regulations and with assistance agencies. Responsible for monthly safety checklists and fire drills. Responsible for acquiring needed items for SIL participants (groceries, funding, medications, etc..) Review on a daily basis and approve daily SDLs (internal progress notes) in Taskmasters. Responsible for reviewing monthly budgeting . Assist in staff training as directed. Will serve as a DSP as needed and responsible for filling shifts for all cases on a daily basis. Must obtain 16 hours of in-service training per year in a job related area as approved by BCSS. Will serve as on-call after normal business hours (on a seven day rotation schedule) for CSS department. CSS Supervisor works a 40 hour week and as needed to meet the needs of the consumer and families. Assist/Volunteer during TARC fundraisers. Accountability to the Program Director and the CEO. Requirements Bachelors degree in human services or related field preferred and a minimum of one post degree work experience in planning and providing services to people with disabilities. Must have reliable transportation, a valid Louisiana drivers license, proof of automobile insurance, social security card, and valid inspection sticker at all times with no laps in coverage. Strong writing skills for documentation, reports, and client communication Must pass a background check and drug screen. Supervisory experience.
