Responsibilities:

Supervise the planning, coordination and implementation of a structured Social Adult Day Care program for seniors with Alzheimer's type dementia and medical frailty. Conduct participant assessments and write care plans Assist with intake screenings and visit days. Maintain communication with all families, and managed care organizations (MMLTC). Provide direct supervision to staff to ensure that the goals of maintaining function, overall safety, and social/emotional support are maximized for all participants. Coordinate staff scheduling in order to ensure adequate staff, including transportation schedules. Supervise food service and ensure that the program is in compliance with all Senior Meal requirements

Qualifications:

High School Diploma required; bachelor's degree preferred. 1 year experience in elder care or related field required. 1 year supervisory experience preferred. Day Haven is looking for a caring and compassionate professional to work well with staff, families and participants. Excellent interpersonal skills are a must. Candidate must possess strong organizational, communication and leadership skills.
