Waiver Support Coordinator

$55,000 per year

Location:

Burnsville, MN Schedule:

Full-time position. This role will begin working on-site , with the potential to move to a hybrid schedule based on performance and operational needs. Occasional travel to other Carefree Living locations may be required.

RESPONSIBILITIES

Monitor waiver expiration dates for residents across multiple Carefree Living locations Track upcoming renewals and notify site leadership of approaching deadlines Coordinate required assessments and ensure appointments are scheduled in a timely manner Communicate with county case workers to confirm requirements and maintain compliance Follow up with site teams to ensure all required documentation is completed and submitted Assist with gathering necessary records and paperwork related to waiver renewals Maintain accurate tracking systems to ensure all waiver steps are completed prior to expiration Work collaboratively with clinical teams and administrative staff to ensure continuity of services and payment Provide updates to leadership regarding waiver status and outstanding items Support process improvement efforts related to waiver tracking and coordination Other duties as assigned

QUALIFICATIONS

Working knowledge of Elderly Waiver (EW), 67/90 waivers, and other applicable waiver programs required Experience working with Medicaid-funded services in assisted living, home care, or similar settings preferred Strong organizational and time management skills with the ability to manage multiple deadlines Excellent communication skills and ability to work collaboratively with multiple teams and external partners Detail-oriented with strong follow-through skills Proficiency with computer systems and Microsoft Office required Ability to travel occasionally between locations as needed Selected candidate must be able to pass a DHS background study
