Supervisory Responsibility:
This position directly supervises the Team member Assistants assigned to work in the day services area.
Location:
Ripon, WI Work Conditions:
This position primarily operates within the company facility, but often involves interacting directly with Team members in various locations throughout the company facility and in the local and nearby communities. Occasional travel by company bus within the community to take Team members on various outings does occur. This position has direct contact with Team members and occasionally provides assistance with activities of daily living (ADLs) such as eating, toileting, and personal cares. Job Summary The Day Services Coordinator coordinates the daily activities and schedule of the day services program, supervises staff, ensure team members are appropriately engaged, and oversee the general operation of the day services program. The Day Services Coordinator provides support for both Day Services staff and Team members.
Job Duties and Responsibilities:

  • Develop, oversee and coordinate individual and group activity schedule that re-enforce daily living skills, consists of recreational, educational and social activities in the company facility and in the community.
  • Supervise day services staff and coordinate staff schedules according to business needs.
  • Provide guidance for Team members as they make life choices and monitor their PCP progress; Recommend and implement adjustments to Team members care and support plan based on their life choices and needs.
  • Maintain effective and consistent communication with the Case Management team and other departments involved in supporting team members to ensure timely updates, collaboration, and continuity of care.
  • Plan and implement care based on each team member's PCP and immediate needs.
  • Provide guidance, coaching, and support to Day Services staff to ensure that strategies and recommendations outlined in team members' PCPs and Training Reports are implemented consistently and effectively.
  • Utilize best practices and person-centered approaches when responding to challenging situations and behaviors. Coach and support Day Services staff in managing these situations calmly, respectfully, and professionally.
  • Create monthly outing schedule and staff weekly schedule. Ensure day services staff are aware of tasks and plan.
  • Address staff concerns in a timely and supportive manner, providing guidance and assistance when conflicts arise among Day Services staff.
  • Promote a collaborative, respectful, and inclusive work environment by fostering teamwork, open communication, and positive working relationships among staff.
  • Approve and submit staff's online timecard, PLT etc.
  • Work cooperatively with all staff throughout the company to achieve department and Company goals.
  • Remain current in and implement new methods of training and education for Team members.
  • Organize Day Services space, equipment and materials. Ensure a clean, tidy and safe environment.
  • Provide direct services and behavioral support to Team members, pending workload, due to staff coverage issues in the day services department.
Assist Team members with activities of daily living (ADLs) including toileting and other personal care activities that are essential to caring for oneself. Use equipment and tools such as a hoyer lift and gait belt to safely move Team members from one location to another.
  • Occasionally transport Team members safely to and from day services activities in the community using personal or company provided vehicles, as necessary.
  • Participate in the Real-Time Feedback and Goal-Achievement Coaching Program at Diverse Options, Inc.
  • Maintain confidentiality policies of the corporation.
  • Other duties as assigned.

  • Proficiently skilled with basic computer applications including Microsoft Office products.
  • Knowledge of how to copy and print documents using a multi-functional printer.
  • Knowledge of how to transmit information or documents to customers, using computers, mail, or facsimile machine.
  • Demonstrated ability to operate light medical equipment, including a hoyer lift and sit to stand.
  • Knowledge of good housekeeping practices.
Education & Experience Required:
Education & Experience Preferred:
  • Bachelors Degree in related field
  • Bachelor's degree in psychology, social work, special education or rehabilitation
  • 3+ years of experience managing day program for adults with disabilities.
  • 5+ years of directly related field
  • 3 years of supervisory experience
Additional Requirements:
At the time of hire, must submit to and pass a caregiver background check, as required by The Caregiver Law under Wis. Stat. § 50.065.
Physical Requirements:
  • Ability to balance, stretch, crouch, bend, stoop, crawl, walk, kneel, twist, climb, stand, and sit.
  • Ability to lift and carry up to 50 lbs.
  • Ability to push and pull up to 200 lbs.
Driving Requirements:
  • Ability to drive and operate an accessible bus and van.
  • Current valid driver's license.
  • Good driving record; must meet company driving standards.
  • Ability to travel independently to various locations.
  • Must be 21 years old in order to drive a Diverse Options, Inc. vehicle.
  • Proof of adequate personal auto insurance is required because the person in this position may be required to use his/her own vehicle for business purposes.
