Derby, VT 05855; St Johnsbury, VT 05819 Apply Overview Salary Range $21.25 - $25.97 Hourly Apply Description Bridge Program Care Coordinator If you're looking for an opportunity to do meaningful work supporting families in your community, join the passionate, community-minded team at Northeast Kingdom Human Services (NKHS). NKHS is seeking a dedicated and detail-oriented Care Coordinator to support families with children and youth (birth to age 22) who have developmental disabilities through our Bridge Program . In this role, you will provide high-quality service coordination designed to promote stability, connect families with critical resources, and support meaningful outcomes for the individuals and families we serve. This is a temporary position with the possibility of becoming permanent based on program needs and performance. This position plays an important role in helping families navigate complex systems of care while ensuring children and youth receive the services and supports they need to thrive. About NKHS Northeast Kingdom Human Services (NKHS) is a 501(c)(3) private not-for-profit organization dedicated to providing high-quality, comprehensive community mental health services in Vermont's beautiful Northeast Kingdom. As a Designated Agency contracted with the State of Vermont, NKHS provides services to individuals with intellectual and developmental disabilities, children and youth experiencing emotional and behavioral challenges, and adults facing mental health and substance use concerns. With more than 500 employees serving nearly 4,000 community members annually across Caledonia, Essex, and Orleans counties, NKHS offers a wide range of programs including case management, community and home supports, residential care, psychiatry, therapy, vocational supports, school-based counseling, emergency care, and respite services. Key Responsibilities Conduct assessments to determine needs and service eligibility Develop individualized care plans that reflect each family's goals and support needs Coordinate and monitor services by collaborating with providers, schools, medical professionals, and community partners Assist families in navigating available resources and ensure timely access to essential supports Research available services and proactively identify resources to support informed decision-making Maintain accurate documentation and ensure compliance with program and regulatory standards Qualifications Strong organizational, communication, and problem-solving skills Ability to work collaboratively with families and multidisciplinary teams Experience in human services, case management, or care coordination preferred Bachelor's degree in human services, social work, psychology, education, or a related field preferred Salary & Benefits $21.25 - $25.97 per hour , depending on education and experience Education reimbursement and loan repayment programs Health and dental insurance 403(b) retirement plan with agency contribution and match Generous paid time off Employee wellness program Additional benefits including life insurance, disability coverage, AFLAC, and an employee assistance program Why Join NKHS At NKHS, our employees are at the heart of the work we do.
