This role serves as the primary point of contact for new residents/clients, ensuring a smooth transition by handling paperwork, verifying eligibility, and supporting the interdisciplinary care team.
Key Responsibilities:
Intake & Onboarding:
Greet new clients and families, conduct initial intake orientations, and assist with completing admission packets.
Documentation & Data Entry:
Maintain accurate client demographic, financial, and medical records in accordance with privacy regulations (e.g., HIPAA).
Eligibility & Verification:
Assist in verifying insurance coverage, state benefits, and background requirements prior to admission.
Coordination:
Liaise with hospital discharge planners, community agencies, and internal case management staff to schedule incoming admissions.
Client Support:
Answer inquiries regarding facility programs, guidelines, and available social support resources.
Qualifications:
Education:
High school diploma required; Associate or Bachelor's degree in Social Work, Human Services, or related field preferred.
Experience:
1+ years in social services, healthcare, or administrative intake roles.
Skills:
Strong communication, empathy, detail orientation, and proficiency with database entry/EHR systems.
