REMOTE PATIENT MONITORING CARE MANAGER
Position Purpose:
Responsible for supporting patients enrolled in a Remote Patient Monitoring (RPM) program through patient enrollment, device education, data monitoring, patient outreach, care coordination, documentation, and escalation of clinically significant findings to the appropriate licensed clinician. The position serves as a key liaison between the patient and the clinical care team. Roles & Responsibilities Patient Enrollment & Onboarding Identify and enroll eligible patients in the RPM program. Assist with obtaining and documenting patient consent. Complete patient onboarding and explain the purpose and expectations of RPM. Educate patients regarding use, maintenance, and storage of RPM devices. Ensure patients understand how and when to obtain and transmit readings. Confirm successful device activation and data transmission. Identify barriers to participation and develop solutions to improve engagement. RPM Monitoring Monitor RPM patient data through the RPM data dashboard. Identify missing, delayed, or abnormal readings according to established protocols. Contact patients when required readings are missing or inconsistent. Troubleshoot device, connectivity, and transmission problems. Maintain accurate records of device assignment and status. Patient Communication & Care Management Conduct monthly telephone or electronic outreach to RPM patients. Reinforce provider-directed treatment plans and self-management strategies. Provide education regarding chronic disease management, medication adherence, nutrition, activity, and other care-plan components as directed. Assess barriers to adherence and patient engagement. Encourage patients to remain active participants in their care. Coordinate routine appointments and follow-up services. Provide appropriate community or social-service resources when identified. Clinical Escalation Follow established RPM alert and escalation protocols. Contact patients regarding readings that meet defined escalation criteria. Obtain relevant information regarding symptoms, adherence, medications, and circumstances surrounding abnormal readings, within the individual's scope of practice. Immediately escalate potentially urgent or clinically significant findings to the designated nurse, APP, or physician. Document all escalations and subsequent instructions. Care Coordination Coordinate communication between patients, physicians, APPs, nurses, specialists, pharmacies, and other members of the healthcare team. Assist with scheduling follow-up appointments and referrals. Support transitions of care following hospitalization or emergency department visits. Identify patients who may require additional clinical or social support. Maintain continuity of communication between the patient and care team. Documentation Accurately document patient contacts, RPM readings, education, interventions, escalations, and follow-up. Maintain current patient demographics and clinical information. Document device distribution, replacement, and return. Ensure RPM activities are appropriately recorded in the EHR and/or RPM platform. Maintain patient confidentiality and comply with HIPAA and organizational policies. Quality & Program Management Track patient participation and adherence. Identify patients with persistent non-transmission or poor engagement. Participate in RPM quality-improvement initiatives. Assist with monitoring program metrics such as enrollment, adherence, alert response, and patient outcomes. Help identify opportunities to improve RPM workflows and patient experience. Participate in staff training and competency assessments related to RPM technology and workflows. Scope-of-Practice Boundary The position should operate under defined clinical protocols and organizational policies. Unless independently licensed and specifically authorized, the MA/Care Manager should not independently diagnose conditions, interpret clinical findings for treatment purposes, prescribe medications, discontinue medications, or change a patient's treatment plan. Workflow Model Referral → Consent → Device Install → Monitor → Escalate → Document → Follow Up The physician, APP, nurse, or other appropriately licensed clinician remains responsible for clinical assessment, diagnosis, treatment decisions, medication changes, and management of significant clinical deterioration. Pay:
From $19.00 per hour Expected hours: 35.0 per week
