Geriatrics Opening in Wisconsin Located in New London/Clintonville Full or Part-time Option

• BE/BC in Internal Med, Family Med or Geriatrics
• Full-time (4-10s) or Part-time options available with flexible schedules!

• Location:

  Clintonville and New London region
• The team consists of 6 Physicians and 10 APCs
• 2-3 SNFs a day (patient schedule built within EPIC)
• On-Call by phone only: 1:11
• 4 weekends per year, Physicians cover holidays 1 per year, back up call by week (just for support for APC if needed), no going in when on call
• Patient census: 8-12
• Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
• 27 Facilities total between skilled nursing and assisted living facilities
• Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve

Senior Care Services:

• Visits completed at patient's location (Nursing Home or Assisted Living)
• Acute visits, compliance visits, & discharge visits for short and long-term residents
• Advance Care Planning & Goals of Care discussions
• Minor procedures at bedside
