Job Description

The Human Services Provider Program Coordinator II serves as the Qualified Intellectual Disabilities Professional (QIDP) within the habilitation services to a specific caseload of residents within the facility. The QIDP is responsible for facilitating the development of the Person Centered Plans (PCP) for each of the residents on his/her caseload and is responsible for supervising the implementation of this treatment plan across three shifts of staff and a variety of daily environments. For residents in the process of transitioning to community based living arrangements, the QIDP is responsible for facilitating the development of and follow-up to the transition plan. This includes ensuring all necessary supports and training are provided prior to discharge. The QIDP is responsible for supervision of the Home Manager assigned to the home and for overall management of the home and all trainer staff working in the home. The QIDP also supervises an Office Assistant as assigned to their home. Minimum Required Education & Experience Bachelor's degree in a human services or related field from an appropriately accredited institution and two years of experience providing professional or direct services (either paid or volunteer) in a setting that serves individuals with disabilities. Management Preferences Documented supervisory experience. Thorough knowledge of the concepts, practices and techniques of the field of intellectual and other developmental disabilities. Documented skills in individualized educational planning, and/ or individual service planning and outcome documentation. Experience managing and supervising the operation of a residential unit and developing cooperative working relationships with the residents, families, staff and communities. Experience coordinating professionals, para-professional and direct support staff in the development, implementation, and evaluation of person-centered services.
Pay:
$41,206.00 - $72,113.00 per year
