Job Description

We are seeking an experienced, results-driven Chief Operating Officer (COO) to lead the day-to-day operations of our growing home care organization while playing a major role in driving patient/member growth and increasing the company's overall census. This is not a position focused solely on managing existing operations. We are looking for an executive who can think like an operator and a business developer — someone who can identify opportunities to bring in new patients, develop referral relationships, create new growth strategies, and build systems that allow the organization to continuously increase its census. The ideal candidate will have strong experience in home care, Medicaid/MLTSS, healthcare operations, business development, referral generation, or a related healthcare environment. Primary ResponsibilitiesPatient & Census Growth Take a leadership role in increasing the company's patient/member census. Develop and execute strategies to consistently bring new patients and referrals into the organization. Build and maintain relationships with hospitals, physicians, social workers, discharge planners, case managers, care managers, MCOs, community organizations, senior centers, and other referral sources. Identify new referral channels and untapped sources of potential patients. Develop creative strategies to increase referrals and convert referrals into active patients. Track referral activity, conversion rates, admissions, and census growth. Establish measurable monthly and quarterly census-growth goals. Create strategies to improve patient retention and prevent unnecessary loss of census. Identify new markets, geographic areas, programs, and service opportunities that can increase the company's patient base. Develop partnerships and community relationships that generate a consistent pipeline of new patients. Work with ownership to develop and implement new business ideas and revenue-generating opportunities. Continuously evaluate what competitors and other successful home care organizations are doing and identify opportunities for the company to improve and differentiate itself. Bring new ideas to the table regularly and turn those ideas into actionable growth initiatives. Operations & Leadership Oversee the company's day-to-day operations and ensure all departments are functioning efficiently. Lead, manage, and hold department managers and operational staff accountable for performance. Oversee caregiver recruitment, onboarding, scheduling, retention, and workforce management. Ensure services are properly authorized, scheduled, documented, and delivered. Monitor billing, claims, collections, and revenue-cycle processes. Work closely with insurance companies, MCOs, case managers, care coordinators, and healthcare partners. Ensure compliance with all applicable federal, state, Medicaid, MLTSS, and home care requirements. Prepare the organization for audits, inspections, accreditation reviews, and corrective action requirements. Monitor key performance indicators (KPIs) and establish measurable goals for departments and employees. Identify operational problems and implement effective solutions. Improve internal systems and processes to increase efficiency and reduce errors. Oversee quality assurance and ensure members receive consistent, high-quality services. Handle escalated employee, caregiver, member, and operational issues. Work closely with ownership on strategic planning, budgeting, growth initiatives, and organizational development. Key areas of performance will include: Increase in active patient/member census Number of new referrals generated Number of new patient admissions Referral-to-admission conversion rate Growth of referral sources and community partnerships Patient/member retention Employee and caregiver retention Operational efficiency Revenue growth Reduction of operational errors and inefficiencies Compliance and quality performance What We're Looking For 5+ years of leadership experience in home care, healthcare, Medicaid/MLTSS, or a related healthcare field. Proven experience growing a healthcare organization, patient census, referral base, or book of business. Strong understanding of home care operations. Experience with Medicaid, MCOs, MLTC/MLTSS programs, or similar healthcare payers strongly preferred. Demonstrated ability to generate referrals and develop new business relationships. Strong network within the healthcare/community referral environment is a major plus. Excellent leadership and people-management skills. Strong business development and relationship-building skills. Excellent communication and negotiation abilities. Strong analytical and problem-solving skills. Highly organized and able to manage multiple priorities. Comfortable working in a fast-paced environment. Ability to establish goals, measure results, and hold teams accountable. Pay:
$76,107.87 - $91,656.78 per year
