The ideal candidate will possess extensive clinical and case management expertise, with a focus on coordinating patient care, discharge planning, and ensuring compliance with healthcare regulations. This role offers an excellent opportunity to contribute to patient recovery and quality of life by collaborating effectively with healthcare professionals and providing comprehensive case management services within a dynamic post-acute care environment. Responsibilities Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to develop personalized care plans. Coordinate inpatient and outpatient services, ensuring seamless transitions between hospital, skilled nursing, and home health settings. Manage discharge planning processes by evaluating patient needs, arranging appropriate post-acute care services, and facilitating communication among multidisciplinary teams. Utilize electronic health records (EHR) systems to document patient information accurately while maintaining compliance with HIPAA regulations. Collaborate with healthcare professionals across specialties such as geriatrics, behavioral health, infectious disease care, and emergency medicine to optimize patient outcomes. Oversee utilization management and diagnostic evaluations to ensure appropriate resource use and adherence to health regulation policies. Engage in patient service activities by providing client communication, education on medical terminology, and support during hospital or clinic visits. Monitor patients' clinical progress in various settings including
ICU, PICU
(Pediatric Intensive Care Unit), medical-surgical units, and post-acute care facilities. Assist in managing complex cases involving trauma center protocols or emergency room scenarios while ensuring timely intervention. Experience Proven experience in clinical case management within hospital medicine, emergency medicine, or post-acute care environments. Demonstrated expertise in critical care settings such as Level I or Level II trauma centers, ICU, or infectious disease units. Familiarity with hospital experience including medical records management, inpatient assessment procedures, and acute care protocols. Prior involvement in discharge planning, home health coordination, hospice care, or managed care programs is highly desirable. Strong knowledge of healthcare policies related to HIPAA compliance, medical terminology, and health regulation standards. Experience working with electronic health records (EHR) systems and managing patient data securely. Ability to communicate effectively with diverse healthcare teams and patients from varied backgrounds in a fast-paced environment. Knowledge of physiology principles relevant to geriatrics, pediatrics (PICU), behavioral health issues, and hospital-based acute care practices is essential.
