— UP TO $20,000 Collierville, TN — Serve residents in your community with flexibility, leadership and uncapped earning potential up to $350,000/year. Position Overview We are seeking a Physician (MD/DO) to serve in a combined role providing on-site skilled nursing facility primary care while functioning as Medical Director , supported through participation in Telos' Certified Medical Director (CMD) training . This role in the SNF setting offers uncapped earning potential and leadership development. Clinical volume is well-supported and manageable, with strong NP/PA coverage and structured after-hours telehealth coverage delivered directly by the provider. Retention Bonus:
Up to $20,000 Fixed Medical Director stipend (separate from RVUs, always applicable) Predictable, manageable clinical volume Strong NP/PA support for routine and regulatory visits Telehealth encounters personally performed by the physician or NP/PA are billable Certified Medical Director (CMD) training fully funded No practice overhead or administrative burden Clinical Responsibilities Provide on-site primary care services to skilled nursing facility residents Evaluate residents for significant changes in condition Collaborate with interdisciplinary care teams and facility leadership Supervise and support NP/PA clinicians providing routine and regulatory care After-Hours Coverage Participate in a defined, structured after-hours call schedule After-hours clinical issues managed via telehealth by the physician or NP/PA on call Telehealth encounters are provider-performed and billable Call volume is predictable, supported, and not overwhelming Medical Director Role & CMD Training Function as Medical Director for assigned skilled nursing facilities Participate in Certified Medical Director (CMD) training Gain experience in: Quality Assurance & Performance Improvement (QAPI) Regulatory compliance and survey readiness Interdisciplinary leadership CMD certification is not required to start All CMD training expenses are fully funded Compensation Clinical Services (on-site & provider-delivered telehealth): Uncapped RVU-based compensation supported by predictable visit volume
Medical Director Role:
