Job Description

Job Summary The Plus Group is seeking a Grants Coordinator to support the Foundation and Community Health programs through grant development, project coordination, community engagement, data management, and outcome reporting. The Grants Coordinator is responsible for researching, developing, and writing public and private grant proposals, as well as managing grant-funded projects from planning through implementation and reporting. This role works collaboratively with hospital departments, community organizations, funders, stakeholders, and other partners to identify community needs, develop programs, secure funding, and measure program outcomes. The ideal candidate is an organized, collaborative, and strong communicator who can manage multiple projects, work with data, develop compelling grant proposals, and build positive relationships with internal and external partners. Key Responsibilities Grant Development & Project Management Research and identify public and private grant funding opportunities aligned with organizational and community health priorities. Develop, write, and coordinate grant proposals, applications, renewals, and supporting materials. Manage grant-funded projects and ensure activities, timelines, budgets, and deliverables are completed as required. Work with internal departments and community partners to develop and plan collaborative programs and funding initiatives. Monitor grant requirements and ensure compliance with funder expectations and organizational policies. Evaluate program effectiveness and progress toward established goals, objectives, and grant deliverables. Data, Reporting & Evaluation Collect, organize, enter, and maintain program and grant-related data using spreadsheets and data collection tools. Track program activities, outcomes, and performance measures. Analyze data and translate findings into clear reports, summaries, presentations, and graphics. Prepare required grant reports and outcome documentation for funders and stakeholders. Identify and communicate community health needs and program outcomes using available data and evidence-based practices. Community Engagement & Collaboration Lead and facilitate meetings, convenings, and discussions with funders, community-based organizations, stakeholders, hospital staff, and other partners. Build collaborative and professional relationships with community members, staff, volunteers, and partnering organizations. Represent and promote Community Health programs throughout the hospital and in community settings. Provide technical assistance and program-related support to internal and external partners as needed. Recognize when program needs fall outside the scope of the role and coordinate appropriate additional resources or services. Program Coordination Coordinate program logistics, including classroom or meeting space, equipment, materials, communications, and other resources. Facilitate program sessions, presentations, updates, and community convenings as appropriate. Support program delivery through effective organization, resource management, and problem-solving. Participate in departmental meetings, staff meetings, trainings, in-services, and interdepartmental work teams. Maintain flexibility and respond effectively to changing organizational and program needs. Professional Standards & Compliance Demonstrate professional, respectful, and effective communication with patients, visitors, staff, leadership, volunteers, community partners, and other stakeholders. Maintain high professional standards related to ethics, knowledge, performance improvement, and ongoing learning. Maintain confidentiality and comply with HIPAA , patient privacy requirements, data security standards, and applicable state and federal regulations. Follow all hospital and departmental policies and procedures. Demonstrate appropriate safety, infection-control, and equipment-use practices. Use hospital resources responsibly and support cost-effective program operations. Maintain reliable attendance, punctuality, and flexibility with assigned work schedules and organizational needs.
Qualifications Required:
Strong written and verbal communication skills. Excellent organizational and project-management skills. Ability to manage multiple priorities, deadlines, and grant deliverables. Strong attention to detail and ability to maintain accurate records. Proficiency with spreadsheets, data collection, and reporting tools. Ability to analyze information and communicate findings through written reports and presentations. Ability to establish and maintain effective relationships with diverse community and organizational partners. Professional judgment, problem-solving skills, and the ability to work independently and collaboratively.
Preferred:
Experience with grant writing, grant administration, or nonprofit/foundation programs. Experience working in healthcare, community health, public health, or a related environment. Experience with program evaluation, outcome measurement, and data analysis. Experience working with funders, community-based organizations, or other external stakeholders. Knowledge of evidence-based community health practices. 20 hours a week $23-$26 an hour. #TPGMOD1

$23.00 - $26.00 per hour Expected hours: 20.0 per week
