Job Description

Bring your clinical expertise and strategic thinking to a role with flexible scheduling! Nye Health Services is seeking an RN MDS Coordinator to lead the MDS process, helping ensure accurate assessments, individualized care planning, and quality outcomes for our residents. This role is ideal for a detail-oriented RN who enjoys connecting clinical information, identifying opportunities, and collaborating with a team to support resident care. What You'll Do Manage the MDS process, including assessment scheduling, completion, coordination, and timely submission. Review clinical records and collaborate with the interdisciplinary team to ensure assessments accurately reflect each resident's needs and services. Support individualized care plans that align with assessment findings and resident goals. Apply clinical judgment and strategic thinking to identify documentation gaps, changes in condition, and opportunities to improve assessment accuracy. Support accurate reimbursement through complete assessments and supporting documentation. Monitor assessment deadlines and maintain organized workflows. Serve as a resource for team members on MDS documentation and assessment requirements. Provide nursing leadership through the scheduled on-call rotation and direct resident care when working additional floor shifts. What We're Looking For A current RN license in the state of Nebraska. Strong clinical assessment skills and the ability to think strategically about resident care, documentation, and the MDS process. Excellent organization, attention to detail, and follow-through. Ability to manage priorities independently and collaborate effectively with nursing, therapy, and other departments. MDS coordination experience and knowledge of assessment requirements and reimbursement processes preferred. Skilled nursing or long-term care experience preferred. RAC-CT certification is preferred but not required. Schedule & Flexibility 24 hours per week dedicated to MDS responsibilities, with flexibility in scheduling to meet assessment deadlines and community needs. Opportunity to work full-time hours by adding a floor nursing shift. Participation in the floor nurse leader on-call rotation every third weekend and every third holiday. What's In It For You? $40.00 - $45.00 per hour Expected hours: 24.0 per week
