We are seeking a dedicated and energetic professional to join our team as a Program Coordinator . This is a unique opportunity to make a meaningful impact by supporting individuals with disabilities in accessing the services they need to achieve greater independence, community integration, and overall life fulfillment. This position requires either a 2-year degree or at least 2 years of experience in the disability services field. Major Responsibilities Support individuals with disabilities in developing and implementing their Individual Support Plans. Facilitate meetings and collaborate with both internal and external team members to ensure support needs are identified and addressed. Maintain accurate and timely documentation to support service delivery and funding requirements. Advocate for individuals to secure and maintain appropriate services and funding. Promote personal growth, independence, and community engagement. 1 FT position available - 40 hrs/wk

Schedule:

Mon - Fri day hours, with on-call every 6 weeks. $22+/hr COC is an
