Job Description
Westside Habilitation Center is seeking a dependable, organized, and compassionate Qualified Intellectual Disability Professional (QIDP) to join our team. The QIDP plays an important role in coordinating services and ensuring that individuals with intellectual and developmental disabilities receive appropriate, person-centered care and active treatment. Responsibilities Coordinate and oversee services for an assigned caseload of individuals with intellectual and developmental disabilities. Participate in and lead Interdisciplinary Team (IDT) meetings. Develop, review, and update Individual Program Plans (IPPs/IHPs) based on each individual's needs, abilities, preferences, and goals. Ensure program objectives and active treatment recommendations are implemented and documented appropriately. Monitor individuals' progress toward goals and make recommendations for changes when needed. Complete required assessments, progress notes, monthly documentation, and other QIDP records. Communicate with families, guardians, nursing, direct care staff, physicians, therapists, and other members of the interdisciplinary team. Conduct routine visits to assigned group homes and monitor the quality of services being provided. Assist with admissions, discharges, transfers, court-related documentation, and placement planning as needed. Ensure services and documentation remain in compliance with ICF/IID regulations, facility policies, and applicable state and federal requirements. Advocate for the health, safety, rights, choices, and quality of life of the individuals served. Provide guidance and training to direct care staff regarding individual programs and identified needs. Participate in surveys, investigations, meetings, and other administrative activities as assigned. Qualifications Bachelor's degree from an accredited college or university is required. Degree in social work, psychology, sociology, education, rehabilitation, human services, or a related field is preferred. Must meet applicable requirements to serve as a Qualified Intellectual Disability Professional (QIDP). Experience working with individuals with intellectual and developmental disabilities is preferred. Strong written and verbal communication skills. Ability to organize and manage multiple responsibilities and deadlines. Strong documentation and computer skills. Ability to work collaboratively as part of an interdisciplinary team. Must possess a valid driver's license and meet facility employment requirements.
