Job Description

Responsibilities:
  • Advocates for individuals' needs and interests through implementation of the interdisciplinary team process and the person centeredperson-centered plan (PCP) according to regulations of Developmental Disabilities Administration (DDA) for all residents.
  • Works cooperatively and collaboratively with each person enrolled in residential program, Residential Supervisors, DSPs, and the Coordinators of Community Services (CCS) to prepare for PCP meetings.
  • Schedules and confirms all PCP meetings with the team in compliance to DDA's published time frame for PCP preparation and meeting times.
  • Collaborates with the person supported and the team to develop goals that are specific, measurable, attainable, relevant, and time-framed (SMART). Writes implementation strategies that are clear and easy to document.
  • Assist as needed to set up ABLE accounts and monitor account activity.
  • Monitor the individuals' income and expenses to prevent assets from exceeding limits required for Medicaid.
  • Presents any rights restrictions recommended by the team for review by the Program Review Committee/Standing Committee.
  • Collaborates with nursing to ensure ratio supports parameters are accurate and consistent with the PCP.
  • Review PCP in LTSS and recommend changes to CCS, notifying Residential Director of any changes that need to be made.
  • Provides PCP training for all Residential Supervisor's and DSP's and documents accordingly. Ensure all documents are in place with -in 30 days of initialcompleted PCP. Meeting.
  • Conduct engagement survey by visiting all houses routinely (1-2 hours per visit) to review documentation on outcomes and goals and provide feedback/training to RS's and DSP's as needed.
  • Implements strategies of goals as needed; Responds to changing needs in a timely manner.
  • Routine demonstration of adherence to principles of person centeredperson-centered thinking and planning; perpetuate values of respect, self-determination, and choice.
  • Reviews all incident reports; Conducts timely investigations in conjunction with incident reports; prepare internals and reportables using the PCIS2 system; monitors trends; makes recommendations to the RD for staff trainings; prepares a PC report on incidents annually as a part of the PC Planning process.
Provide copies of all incident reports to Administrative Assistant to Residential Director.
  • Provides incident reports (internal and reportable) to management for review by the Program Review Committee/Standing Committee.
  • Liaison to the Behavior Specialist; responsible for collecting BP data and scheduling trainings as needed on BP's; provides training on BP's as delegated by the Behavior Specialist.
  • Attends trainings as requested and shares information with staff.
  • Monitors, reports, and follows through with any concerns regarding daily appearance or any observed unmet needs of people supported.
  • Maintain routine communication with families and advocates of the individuals.
  • Acts as liaison between community and CGH; represent CGH professionally at all community events and center meetings.
  • Provides updates as needed in a proactive manner as well as in response to concerns or crises, to the Residential Director (RD), the people supported, CCS, family, nurse, residential and day staff.
  • Be on-call and respond to on-call situations, as needed. Provide timely and thorough updates to the RD and support team.
  • Monitor the individuals' income and expenses to prevent assets from exceeding limits required for Medicaid; work with the team proactively to identify and plan for large expenses as needed.
  • Assist individuals in planning weekend activities and vacations; coordinating with staff to plan for activities. Update the RD on planned activities and follow the Vacation Planning Protocol.
  • Updates the RD on all Leaves of Absence (LOA's); Coordinates plans with the nurse, residential supervisor, and vocational staff for LOA's.
  • Collaborates with the RD on Quality Enhancement (QE) Goals; takes the lead on implementing and documenting QE goals.
  • Make sure goals are implemented and documenting monthly, go over with staff.
  • Works collaboratively and cooperatively with the Program Assistant and Administrative Assistant to RD and all other Team Members.
  • Work hours will be flexible based on priority needs of CHG, the needs of the individuals, and on inclement weather. As well as working 1- Late Day, 1 Early Day and 1 Saturday or Sunday a month visiting allentire case loads homes.
  • Engagement Surveys Monthly
  • DSAT Monitor the individuals' income and expenses to prevent assets from exceeding limits required for Medicaid with program coordinator.
  • Account for Wal-Mart, Acme and Vise charge cards, track users and review receipts.
  • Review Individuals account balances to ensure a $200 balance is left after check requests.
  • Maintain up to date records including removal of designated records from main record and dead file prior to annual PCP meeting; update face sheet annually and as needed.
  • Scan records as needed.
  • File in binders monthly, hanging file monthly, and other as needed.

Act as liaison between community and CGH; represent CGH professionally at all community events and center meetings when community members are present, as directed.
  • Necessary to maintain confidentiality concerning all records.

  • Attend meetings as requested by the Residential Director.
  • Work hours will be flexible based on priority needs of CGH, the needs of the individuals, and on inclement weather.
  • Necessary to maintain confidentiality concerning all records.
  • Work collaboratively and cooperatively with all TEAM members.
  • Other duties as assigned.
Essential Requirements of Work:
  • Knowledge of the interdisciplinary team process and individual program plans as outlined in DDA regulations.
  • Excellent written and verbal communication skills to include accuracy of information communicated required for sound decision making.

  • Exhibits teamwork through effective working relationships with other staff members; provides assistance to other departments as needed.
  • Ability to manage multiple projects and set priorities through efficient time management to meet deadlines and maintain compliance with PCP's.
  • Knowledge of community resources and ability to use effectively.
  • Ability to communicate courteously with families of individuals served by the program.
  • Experience on Microsoft Office and internet usage required. Necessary to maintain confidentiality concerning all records.
  • Maintains a professional working relationship with co-workers at all times.
  • Provide a written schedule of weekly task to be completed.
Nature of Work:
  • The employee in this classification receives general direction from the Residential Services Director; changes in work hours or schedule to be approved by Director.
  • Hours will be flexible based on needs of residents and job duties and includes occasional evenings and weekend hours.
Work is evaluated in terms of overall professionalism, ability to meet deadlines, and adherence to regulations and policies.
Pay:
$24.00 - $27.00 per hour Expected hours: 40.0 - 50.0 per week
