Come Make a Difference With Us! Do you love building relationships, helping people learn, and turning everyday moments into meaningful experiences? Lovely Cares is seeking an energetic, compassionate Program Lead to support a young adult with ASD who lives in his own apartment, has a great laugh, and loves to have fun. This is an active, engaging role for someone who enjoys swimming, long walks, dining out, community outings, and participating in family activities. We're looking for someone who is comfortable being involved, creating meaningful experiences, and supporting an active lifestyle. If you bring curiosity, patience, creativity, leadership skills, and a genuine desire to help someone grow, we want to meet you! About the Opportunity Lovely Cares has been serving individuals and families for five years, built around a simple belief: great support starts with truly knowing the person. Our founders bring more than 10 years of human services experience, including work in larger agencies, where we saw how important it is to make space for genuine connections, individual interests, and each person's goals. Grounded in our backgrounds in Health Service Management and Health Informatics, hands-on work in school settings with children with and without disabilities, and extensive direct support for adults with disabilities, we established a small, person-centered agency dedicated to putting the individual first rather than relying on a one-size-fits-all approach. In this unique opportunity, you will partner closely with the individual and his parent/guardian, alongside a collaborative multidisciplinary team of professionals, to support his independence, learning, and lifestyle. As the Program Lead, you will coordinate daily services, support direct care staff, and ensure individualized goals are consistently carried out. What You'll Be Doing Person-Centered Support & Activities Plan and lead engaging, person-centered activities that align with the individual's support plans, interests, and personal goals. Actively participate in swimming activities, including entering the pool with the individual. Accompany the individual on long walks, including outings in Wayzata, and support other outdoor and community activities. Plan and coordinate restaurant dining experiences, social outings, and family activities. Support exercise routines, physical activity, and wellness goals according to the individual's support plan. Maintain appropriate, professional, activity-specific attire and respectful conduct in public, recreational, and family settings. Daily Living, Nutrition & Medication Support Prepare meals and snacks according to the individual's dietary guidelines, preferences, and nutritional needs. Develop meal preparation plans and maintain organized, accurate grocery lists. Monitor food inventory, organize refrigerator and pantry items, and assist with grocery shopping. Follow individualized dietary instructions, including holistic or specialized meal plans when applicable. Administer medications according to authorized medication orders, the individual's support plan, required training, and Minnesota 245D requirements. Maintain accurate medication administration records and promptly report medication-related concerns. Leadership, Documentation & Data Collection Coordinate daily operations, staff activities, schedules, and implementation of individualized support plans. Demonstrate proficiency in Microsoft Excel and Word to create spreadsheets, reports, schedules, and professional documents. Collect, organize, and maintain accurate data related to daily activities, progress toward goals, behavioral observations, and support outcomes. Track progress, identify patterns, and communicate findings to the Program Lead's supervisors and multidisciplinary support team. Maintain organized electronic records, documentation, and data collection systems. Utilize visual supports, Augmentative and Alternative Communication (AAC), and Positive Behavior Supports. Support staff training, model professional practices, and maintain consistent communication with the parent/guardian and support team. Ensure compliance with applicable Minnesota 245D requirements and company policies. Who You Are An upbeat, dependable, organized multitasker with patience, positive energy, and a sense of humor. Comfortable with swimming, extended walks, exercise, dining experiences, and family activities. Skilled in Microsoft Excel, Word, electronic documentation, and accurate data collection. Organized and detail-oriented when managing schedules, records, medication documentation, meals, and grocery lists. Experienced in supporting individuals on the autism spectrum and familiar with AAC, visual supports, mentoring, or teaching. Interested in holistic healthcare, functional medicine concepts, nutrition, and individualized wellness approaches. A proactive leader who communicates effectively, solves problems, and works collaboratively with families and professionals. Flexible, creative, and motivated to learn new approaches. Required Qualifications College degree or equivalent education and experience. Experience supporting individuals with autism or developmental disabilities.
