FULL-TIME SERVICE COORDINATOR

What Does an Early Intervention Service Coordinator Do? As a Service Coordinator, you become an important point of contact for families participating in the New York State Early Intervention Program . You'll help families understand their rights, navigate the Early Intervention process, access appropriate services, and coordinate the professionals involved in supporting their child's development.
Responsibilities Include:
Guide families through the Early Intervention process Help families understand their rights and available services Participate in the development, review, and implementation of Individualized Family Service Plans (IFSPs) Coordinate and monitor authorized Early Intervention services Communicate with families, providers, evaluators, and Early Intervention personnel Monitor case progress and service delivery Maintain timely, accurate, and compliant Service Coordination documentation Manage assigned cases, deadlines, and required follow-up Participate in IFSP meetings, supervision, training, and team meetings Follow NYS Early Intervention requirements and Therapists 2 Go policies Support families through remote, home, and community-based activities as required Experienced SCs AND New Career Candidates Welcome We are looking for two types of candidates: Experienced Early Intervention Service Coordinators who are looking for a supportive agency, flexibility, and an opportunity to grow their caseload. Minimum Qualifications Bachelor's degree in Psychology, Sociology, Social Work, Human Services , or a related Health and Human Service Field
• Strong written and verbal communication skills Strong organization and time-management skills Ability to independently manage responsibilities and deadlines Ability to manage a multi-family caseload Comfortable communicating with families and professionals Willingness to meet families in their homes and/or community when required Ability to work independently in a mostly remote environment Dedicated home workspace Computer or laptop Reliable internet and telephone access Printer/scanner or equivalent document-management capability

This is not a traditional salaried position . Earnings are generated through eligible billable Service Coordination activities. The position is mostly remote , providing significant flexibility, while still requiring Service Coordinators to participate in home, community, and other in-person activities when necessary.
