Job Description

Pulmonary Program Coordinator (RN)
Salary:
$72,800- $83,200 annually (depending on experience)
Hourly Equivalent:
$35- $40
Shift:
Full-Time Autumn View Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs. RESPONSIBILITIES
Oversee and coordinate daily operations of the Pulmonary Program Coordinate individualized care plans for residents with pulmonary and respiratory needs Monitor residents with respiratory and pulmonary conditions Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams Assist with admissions, assessments, and care coordination for pulmonary residents Provide staff education on pulmonary care and clinical best practices Ensure compliance with regulatory and clinical standards Participate in quality improvement and program development initiatives
REQUIREMENTS
Active New York State Registered Nurse (RN) license required Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred Strong clinical assessment and care coordination skills Experience with care planning and interdisciplinary collaboration preferred Strong leadership, organizational, and communication skills
