Oversee daily program implementation and service delivery, monitoring progress, adjusting plans, and ensuring supports, safeguards, protective oversight, and meaningful activities are provided. Advocate for individuals' rights, health, safety, and quality of life, identifying needs, reporting concerns or incidents, and ensuring compliance with mandated reporting and human rights requirements. Maintain accurate clinical and program documentation, including goals, records, evaluations, medical and program data, psychosocial histories, and regulatory requirements while protecting confidentiality. Coordinate with families and interdisciplinary teams, serving as the primary family contact and participating in admissions, discharges, psychiatric reviews, Human Rights Committee meetings, and benefits coordination. Support and supervise program staff through orientation, training, mentoring, safety monitoring, and ensuring adherence to agency policies, OPWDD requirements, and applicable federal and New York State regulations.

We require:

Bachelor's Degree in Human Services or related field Valid & Qualified New York State Driver's License At least one (1) year of experience working with I/DD population Excellent verbal & written communication skills QIDP experience preferred Physical Aspects may include but are not limited to lifting and transferring of an adult/child participant.
