Not Forgotten Home and Community Services is hiring a Program Specialist to assist its Residential Team. The Program Specialist, under the supervision of the Director of Operations, will oversee the programming of residential services. The Program Specialist will implement person-centered practices to promote community integration and support individual choice. This role focuses on delivering person-centered programming while ensuring compliance with state standards, including Chapters 2380, 6100, and 6400. Key responsibilities include developing processes, educating staff, supporting and conducting audits, and integrating regulatory expectations across all service levels.

ESSENTIAL FUNCTIONS

Coordinate the

Completion of Assessments :

Ensure that all required assessments are completed thoroughly and on time. This includes scheduling assessments, collecting necessary data, and collaborating with other professionals involved in the assessment process. Participate in the

Individual Plan Process :

Collaborate with the individual, their families, and the support team to create a comprehensive plan that addresses the unique needs and goals of each individual.

Provide and Supervise Activities :

Design and supervise a variety of activities tailored to meet the individual needs of clients as outlined in their particular plans. Ensure that these activities promote skill development, engagement, and overall well-being. Participate in

Circles of Support :

Take part in Circle(s) of Support to facilitate personal future planning for individuals. This includes discussions about dreams, goals, and necessary support to help the individual achieve their desired outcomes.

Monitor Appointments and Follow-Ups :

Keep track of individuals' appointments, ensuring timely follow-ups. Attend appointments when necessary to provide support and assistance to the individuals during these visits.

Support Day-to-Day Living Concerns:

Offer support to individuals with their everyday living challenges, ensuring that any concerns raised are addressed promptly and effectively, promoting their independence and quality of life.

Identify Service Delivery Needs :

Recognize and assess service delivery needs and weaknesses. Actively participate in formulating policies and procedures designed to enhance service delivery effectiveness and efficiency.

KNOWLEDGE, SKILLS, AND ABILITIES

: Ability to administer private, nonprofit service organizations, including an understanding of the legal responsibilities and constraints of such organizations. Ability to obtain and follow both Chapter 6400 and Chapter 6100 regulations. Ability to establish and maintain effective working relationships with federal, state, regional, and local agencies, community leaders, and the general public. Knowledge of philosophy of IDD and current concepts on IDD. Knowledge of available public and private resources and services for the IDD. Knowledge of federal and state laws pertaining to aging and related services. Knowledge of modern principles, methods, and techniques of administration and program planning.

EDUCATION AND EXPERIENCE

A Program Specialist shall have one of the following groups of qualifications: A master's degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism. A bachelor's degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism. An associate degree or 60 credit hours from an accredited college or university, and 4 years of work experience working directly with individuals with an intellectual disability or autism. Pay:

$55,000.00 - $60,000.00 per year
