$17/hr. Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties.
Responsibilities:
To provide opportunities for Community Connections Program clients to engage in new experiences, create relationships, support to develop potential friendships and contribute to their community based on their interests and preferences. Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion. Participates in the individual plan process including development, team reviews and implementation, in accordance with applicable PA Code 6100. Supports the communication needs of the individuals and promotes involvement with families and friends with assistance from the Community Connections Program Coordinator. Assists with the identification, pursuit, scheduling, and coordination of opportunities to develop and retain relationships between CCP clients & community members. Provide peer mentoring and coaching to newly hired staff on the needs of the individuals supported as identified in the plan. Assists the Assistant Director of Community Connections Program in providing, coordinating, and overseeing activities for the individuals in accordance with the individual plans. Supports the integration and engagement of individuals in the community. Complete Daily Service Notes and goal documentation and reviews for completion and accuracy within the service day. Assists the CCP program with identifying and maintaining community involvement activities for clients Problem-solve and participate in day-to-day crisis management. Provides direct supervision of clients at required ratio during scheduled program hours. Provides behavioral support including execution of individual SEEPs and Behavior Plans as written. Provides required supports for personal care and safely lifting / transferring clients as needed. Works as a cooperative member of the team to promote positive, pleasant, efficient work environment. Takes initiative with non-routine tasks. Participates in team meetings, teambuilding activities, and trainings as scheduled. Transports clients, in agency vehicles, as needed and as appropriate. Addresses any unmet safety or welfare needs of the clients apparent during program hours. Other duties as assigned.

EOE Qualifications:
High school education One year of experience working with people with disabilities or another care-giving role, preferred. Ability to lift and transfer.

WORKING CONDITIONS
100% Community based and direct care provided to clients with disabilities Transport of clients into the community during program hours using agency vehicles Program hours are Monday - Friday 8:00am-3:30pm Additional hours scheduled for staff training and meetings
REQUIRED LICENSES/CERTIFICATES
Valid PA driver's license Acceptable Motor Vehicle Reports results Act 34 clearance and FBI background check, if applicable CPR / First Aid certification 24 hours of agency provided training per year
