TriSource is seeking dedicated Program Specialists/Case Managers to support an IDD non-profit organization located in Norristown, PA. In this role, you will lead the charge in developing and implementing comprehensive, individualized programs that maximize the medical, intellectual, social, and emotional well-being of the residents. You will collaborate with families and community resources to ensure regulatory compliance.

Schedule:

Monday-Friday 8a-4p

Pay Rate:

$25/hour

Responsibilities:

Caseload:

Manage a caseload of 25-30 clients

Lead Individualized Care:

Serve as the team leader in developing, coordinating, and evaluating Client-Centered planning goal strategies

Monitor & Evaluate Plans:

Assess individual program plans to ensure compliance with state and agency regulations, covering written programs, financial/banking plans, medications, and general well-being

Support and Train Staff:

Provide regular training, perform monthly home/individual visits, and mentor direct service staff to resolve operational challenges

Coordinate Transitions:

Manage individual placement, transfer, and discharge planning according to agency and government regulations Act as

Primary Liaison:

Build strong partnerships with families, day programs, and community services to track progress and anticipate needs

Leadership Collaboration:

Actively participate as a member of the management team to solve complex problems and plan for future operational opportunities

Flexible Coverage:

Perform assigned evening, weekend, direct service, and on-call duties

Requirements:

Bachelor's Degree in Human Services (or related field) Case management experience is preferred Experience working with children Valid Driver's License, personal vehicle, and proof of auto insurance Active CPR/First Aid Ability to list up to 50 lbs Apply today!
