Manages housing and transportation initiatives in alignment with contract requirements and transition goals, coordinating day-to-day activities, maintaining workflows, and escalating urgent or recurring barriers that could affect member safety, transition readiness, service access, or continued community living Builds partnerships with housing authorities, landlords, transportation providers, public agencies, and community organizations Coordinates the resources needed to address affordability, accessibility, eligibility, documentation, and service barriers while supporting member choice and housing stability Monitors contract deliverables, project expenses, housing and transportation measures, transition outcomes, and service access Analyzes trends and recurring barriers Coordinates required reporting, corrective actions, and cross-functional process improvements Provides day-to-day direction and coaching to team members Represents the company in assigned client and stakeholder meetings Maintains productive relationships Completes other assigned duties and special projects
Required Skills and Experience:
Bachelor's degree in healthcare administration, public health, social work, human services, housing or community development, business or public administration, transportation planning, or a related field; equivalent training and experience may be considered Five years of related experience, including program or project management in healthcare, Medicaid, human services, housing, transportation, community development, or social services Demonstrated ability to manage initiatives, build stakeholder partnerships, communicate effectively, coach team members, use standard computer applications Ability to travel locally or overnight up to 50%
Preferred Skills and Experience:
Master's degree in a related field Experience with housing placement and stability, landlord or housing-authority partnerships, transportation resources, community transitions, or services for individuals with complex needs Experience with quality improvement, data analysis, process improvement, and Medicaid or other government-sponsored programs Relevant housing, fair housing, accessibility, supportive housing, transportation, or community-development training or certification Bilingual English/Spanish skills are a plus
Salary Range :
93000 - 115000 USD / year
