Client Care Coordinator / Scheduler

•Job Description

POSITION PURPOSE

The Client Care Coordinator / Scheduler is responsible for the coordination of services between clients and caregivers for the implementation of schedules, adequate staffing and continuity of care to best meet the needs of the clients.

PRINCIPAL ACCOUNTABILITIES

 Answer the telephone and greet the public.  Demonstrate dependability.  Recruit and advertise for direct care staff.  Process applications and check references, run Criminal Background Checks for all potential employees. Schedule interviews for potential employees with appropriate supervisor. Ensure TB tests are obtained upon hire and annual testing records maintained.  Conduct new caregiver staff orientation.  Schedule caregiver staff for hours of service on new clients and open hours/shifts on existing cases. Provide consistency in caregiver scheduling.  Maintain computer schedules and ensure timely data entry for schedules and client information.  Ensure that all employee files are complete and enter the appropriate information into the scheduling system.  Maintain documentation on all current and past clients.  Monitor the telephony logs on a daily basis. Counsel staff regarding tardiness and correct use of the system. Make the appropriate corrections to the system to facilitate accurate processing of payroll.  Ensure timely copying and filing of appropriate caregiver and client documentation.  Participate in the 90-day and annual evaluations of caregiver staff.  Recognize and reward good job performance and promote caregiver recognition.  Maintain positive relationships with all clients and referral sources.  Independently respond to and resolve any clients' complaints or concerns.  Communicate and refer appropriate matters to the Director for direction.  Maintain absolute confidentiality of all information pertaining to employees, clients and client's families.  Communicate and reinforce Visiting Angels' policies and procedures, Basic 12 and the Points System to caregiver staff.  Maintain and order office supplies, as needed.  On-call evenings and weekends, as directed. Keep on-call materials current with client information, instructions and directions, current roster of care and roster of active direct care staff.  Perform other functions as deemed appropriate by the management team.  Demonstrate dependability.  Maintain a safe environment for clients.  Educate client/responsible party as directed by the Registered Nurse.  Assure that the client and family understand the home care plan and their responsibilities.  Teach household members who are responsible for assisting the client with his/her personal care needs.  Serve as an advocate for the client by communicating and collaborating with other health service personnel and community based service providers, as delegated.  Maintain accurate and timely documentation in preparing client visit and progress notes with follow up.  Inform the Registered Nurse of changes in client's condition and/or needs.  Maintain compliance with state regulations affecting home care.  Demonstrate knowledge of current health practices when performing home care services.  Perform related duties and responsibilities as deemed appropriate by the management team.  Maintain absolute confidentiality of all information pertaining to employees, clients, and client's family members.

SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES REQUIRED

 High school diploma and two years of experience in an office setting, preferably in heath or home care.  Demonstrate proficiency with Microsoft Office (Word, Excel and Outlook) applications, scheduling systems and other health care industry related software.  Ability to listen and communicate clearly, fluently and diplomatically orally and in writing.  Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image.  Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, as well as, work independently with a minimum amount of direction and/or supervision.  Ability to generate goodwill for the Agency with staff, clients, their family members and referral sources. Demonstrate a strong commitment to client service excellence.  Ability to lawfully work in the U.S.
