Position:
Care Coordinator The Care Coordinator plays a vital role in delivering the "best in-home care" by managing care plans that support both clients and their families. You'll be responsible for ensuring each interaction adds value, builds trust, and leads to extraordinary care experiences.
Key Responsibilities:
Act as the first point of contact: Answer incoming calls promptly with compassion and professionalism, schedule in-home consultations, and initiate care services. Conduct in-home assessments and develop personalized care plans based on client needs. Convert inquiries into clients through skilled communication and a service-oriented approach. Build and maintain strong relationships with clients, families, caregivers, and referral sources. Monitor care plans to ensure safety, effectiveness, and client satisfaction. Mentor caregivers and support their ongoing development and performance. Multitask efficiently and remain calm and positive in a dynamic office setting.
What We're Looking For:
High school diploma or equivalent; at least 2 years of business experience Previous experience in home care or healthcare preferred Sales or customer service experience with a proven ability to convert leads over the phone Exceptional communication and organizational skills A "people person" with a passion for service Ability to work independently and exercise sound judgment Availability for occasional on-call duties outside regular office hours Valid driver's license and access to a reliable, insured vehicle How to
