Private Client Care Manager Location:
Randolph, NJ Schedule:
Approximately 3 visits per week + medical appointments Compensation $70 per client visit (up to 1.5 hours) $45/hour for time beyond 1.5 hours $45/hour for administrative and care coordination work Federal mileage reimbursement rate when transporting the client in your personal vehicle to medical appointments Are you an experienced RN looking for a flexible, meaningful part-time nursing role without the pace and demands of a hospital or facility setting? BrightStar Care of Morris & Essex County is seeking a compassionate, highly organized Registered Nurse (RN) to serve as a Care Manager and Medical Advocate for one independent 80-year-old client in Randolph, NJ. This is not a traditional bedside nursing position. You will develop an ongoing relationship with one client and serve as a trusted healthcare partner—helping coordinate her care, accompanying her to medical appointments, managing medications, advocating on her behalf, and keeping her family and our Director of Nursing informed. This is an excellent opportunity for an experienced RN seeking meaningful supplemental work with a limited weekly commitment and some scheduling flexibility. What You'll Do As the client's RN Care Manager, you will: Visit the client approximately 3 times per week Monitor the client's overall health and identify changes or concerns Organize and manage weekly medications and pillboxes Schedule and coordinate physician appointments, diagnostic testing, referrals, and other healthcare services Accompany the client to physician appointments, procedures, and other healthcare visits Transport the client to appointments using your own reliable vehicle when needed Serve as the client's advocate when communicating with physicians and other healthcare providers Document physician recommendations, treatment plans, medication changes, and follow-up needs Provide clear verbal and written updates to the client's family following medical appointments Communicate clinical observations, changes in condition, and concerns to the BrightStar Care Director of Nursing Collaborate with the Director of Nursing regarding changes in care needs, medications, and recommended interventions Coordinate follow-up appointments and help ensure recommendations are carried out Help the client and family successfully navigate the healthcare system Provide occasional additional support when an urgent client need arises Why This RN Position Is Different This position is ideal for an RN who enjoys nursing but is looking for something different from a traditional hospital, facility, or shift-based position. You'll have the opportunity to: Focus on one private client Build a meaningful, ongoing relationship with the client and her family Use your nursing experience for care management, medical advocacy, and healthcare coordination Work independently while having the support of an experienced Director of Nursing Maintain a limited part-time schedule Be compensated for administrative and care coordination time in addition to client visits Help an older adult maintain her health, safety, and independence Schedule & Availability The typical schedule includes: Approximately 3 client visits per week Visits are generally up to 1.5 hours Additional paid time when visits extend beyond 1.5 hours Additional paid administrative time for care coordination and related responsibilities Additional hours when accompanying the client to medical appointments Some scheduling flexibility based on the client's needs and physician appointment availability Occasional availability when an urgent client need arises Because medical appointments may occur during normal business hours, candidates should have some weekday/daytime flexibility.
Qualifications Required:
Active New Jersey Registered Nurse (RN) license Valid driver's license and reliable vehicle Strong nursing assessment, communication, and organizational skills Ability to communicate confidently and professionally with physicians and other healthcare providers Strong documentation skills and attention to detail Dependable, proactive, compassionate, and comfortable working independently Comfortable transporting the client to medical appointments when needed Comfortable working in a home with cats Ability to maintain client confidentiality and professional boundaries
Preferred:
Experience working with older adults Experience in home care, geriatrics, care management, case management, or patient advocacy Training & Support We want the transition to be seamless for both the RN and the client. The selected RN will receive individualized training and a comprehensive handoff from the client's current RN Care Manager to ensure continuity of care. You will also work closely with and receive ongoing support from the BrightStar Care Director of Nursing.
