Are You Great at Building Relationships & Helping People? Are you looking for a full-time role where you can support clients and families while developing relationships with healthcare professionals and community organizations? As a Client Care & Referral Relations Coordinator, you'll help guide clients through the care process while representing WJM Home Care throughout the Melrose community. This position includes client coordination, referral development, community outreach, local travel, and occasional community events outside regular business hours. - Before You Apply! Do You Qualify? You have experience in client service, care coordination, community relations, business development, senior services, healthcare, hospitality, or a related field? You have strong communication, relationship-building, and organizational skills? You can manage multiple priorities, maintain accurate records, and follow through on time-sensitive commitments? You are comfortable using CRM, scheduling, email, presentation, and standard office technology? You have a valid driver's license, reliable transportation, required automobile insurance, and the ability to travel locally? - Position Details Full Time Melrose, Massachusetts & surrounding service area Local travel required Occasional community events outside regular business hours Bonus and incentive programs included Not a work-from-home position - What You'll Do Respond to prospective clients, current clients, families, referral partners, and community contacts. Gather and document client care needs, preferences, schedules, and service expectations. Coordinate consultations, assessments, service starts, reassessments, and follow-up. Conduct client satisfaction and quality check-ins and help address concerns or changing needs. Work with scheduling, recruitment, operations, and caregiving teams to help coordinate client needs. Build relationships with hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, physician practices, home health and hospice agencies, elder-law attorneys, geriatric care managers, councils on aging, and other community organizations. Conduct in-person visits, meetings, educational presentations, community events, and follow-up outreach. Track referral activity and maintain accurate relationship and follow-up records.
