Job Description

Purpose:
Is responsible for assessing new clients and reassessing existing clients. May be asked to participate in the packaging of congregate and home delivered meals as necessary.
Responsibilities:
Use an Area Agency on Aging prescribed form to assess potential clients as assigned by the Prime-Time Office Staff Use an Area Agency on Aging prescribed form to reassess clients as assigned by the Prime-Time Office Staff Notify the Director or Prime Time Office Staff of any client found to be at high risk or of a change in any client's status Assist Prime Time Office Staff in the preparation of client files Package food for distribution by home delivered mean staff Other duties as assigned
