POSITION SUMMARY
Mind & Mane, LLC is seeking a detail-oriented and experienced Qualified Mental Health Professional (QP) / Person-Centered Plan (PCP) Documentation Writer to support the development, completion, revision, and ongoing maintenance of high-quality clinical and service-planning documentation. The primary responsibility of this position is to develop individualized, person-centered, clinically appropriate, and audit-ready Person-Centered Plans (PCPs) and related documentation using information obtained from Comprehensive Clinical Assessments, service authorizations, treatment recommendations, client interviews, care team input, and other supporting records. The ideal candidate understands North Carolina behavioral health services, Medicaid documentation expectations, person-centered planning, measurable goal development, medical necessity, and the importance of ensuring that documentation accurately reflects the individual rather than relying on generic or duplicated language.
PRIMARY RESPONSIBILITIES
The QP/PCP Documentation Writer will: Develop initial, annual, updated, and revised Person-Centered Plans (PCPs) based on the individual's assessment, diagnoses, strengths, needs, preferences, risks, goals, and recommended services. Review Comprehensive Clinical Assessments (CCAs), diagnostic information, previous PCPs, service recommendations, authorizations, and other clinical records before developing plans. Translate assessment findings into individualized goals, measurable objectives, interventions, strategies, and desired outcomes . Write person-centered statements using language that reflects the client's voice, communication style, preferences, and level of understanding. Ensure goals and interventions directly correspond to identified needs and the services requested or provided. Develop PCP documentation for applicable Mind & Mane services, including Peer Support Services, 1915(i) services, outpatient behavioral health services, substance use services, and other programs within the employee's qualifications and assigned responsibilities . Complete PCP updates when the individual's condition, needs, goals, services, living situation, risks, or treatment recommendations change significantly. Obtain required signatures, dates, service information, provider information, and supporting documentation, or route them to the appropriate team member. Identify missing, inconsistent, contradictory, or insufficient information and communicate with the appropriate clinician, QP, supervisor, or administrative staff for clarification. Collaborate with therapists, QPs, Peer Support staff, substance use professionals, care managers, and other members of the treatment team when additional information is required. Ensure documentation demonstrates medical necessity and continued service need when applicable. Assist with documentation needed for service authorization and reauthorization requests. Complete documentation within established agency and payer deadlines. Maintain accurate documentation in Mind & Mane's designated electronic health record and/or documentation systems. Participate in documentation training, quality improvement activities, chart reviews, and corrective action activities when needed. Maintain compliance with HIPAA, confidentiality requirements, agency policies, Medicaid requirements, applicable MCO requirements, and other regulatory standards. Perform other documentation and quality-assurance duties within the scope of the position as assigned.
DOCUMENTATION STANDARDS
All documentation completed by the QP/PCP Documentation Writer must be:
Person-Centered:
Plans must reflect the individual's actual needs, goals, preferences, strengths, barriers, and voice.
Individualized:
Copying and pasting generic goals or interventions between clients is prohibited.
Measurable:
Objectives should include observable or measurable indicators of progress whenever appropriate.
Assessment-Driven:
Goals, interventions, and services must be supported by information contained in the assessment and other appropriate clinical documentation.
Consistent:
Diagnoses, needs, goals, service recommendations, frequency, duration, and interventions should be consistent across applicable records.
Timely:
Complete documentation according to Mind & Mane, payer, authorization, and regulatory deadlines.
Audit-Ready:
Documentation must be sufficiently clear and complete to withstand internal QA review, MCO review, Medicaid audit, accreditation review, or other regulatory review.
REQUIRED QUALIFICATIONS
Candidate must meet applicable requirements to function as a Qualified Professional (QP) for the services/documentation assigned to the position. Related human-services field that meets applicable QP requirements. The candidate must also have the required combination of education and professional experience for the assigned populations and services.
PREFERRED QUALIFICATIONS
Preference will be given to candidates who have: Previous experience writing Person-Centered Plans or treatment/service plans. Experience with North Carolina Medicaid behavioral health services. Experience working with an NC LME/MCO or Tailored Plan. Experience with Peer Support Services and/or 1915(i) services. Experience completing service authorization or reauthorization documentation. Knowledge of behavioral health diagnoses and functional impairments. Experience reviewing Comprehensive Clinical Assessments. Knowledge of mental health and substance use disorder services. Experience with Medicaid audits, utilization review, quality assurance, or clinical documentation review. Strong understanding of SMART goals and person-centered planning. Experience with electronic health record systems.
REQUIRED SKILLS
The successful candidate must demonstrate: Excellent clinical and professional writing skills. Strong attention to detail. Ability to identify inconsistencies between assessments, PCPs, authorizations, and progress documentation. Ability to transform clinical assessment information into understandable person-centered language. Strong organization and time-management skills. Ability to manage multiple PCPs and documentation deadlines simultaneously. Ability to communicate professionally with clinical and administrative team members. Understanding of HIPAA and client confidentiality. Ability to receive feedback and make timely documentation corrections. Ability to work independently while meeting agency quality standards.
WORKING RELATIONSHIPS
The position works collaboratively with: Clinical Director Chief of Staff Qualified Professionals (QPs) Licensed Therapists Peer Support Specialists Substance Use Professionals Authorization/Billing Team Administrative Staff Care Managers and external treatment-team members, as appropriate

$20.00 - $25.00 per hour Expected hours: 10.0 - 25.0 per week
