DIABETES CARE & EDUCATION SPECIALIST

Job Description

ESSENTIAL FUNCTIONS
1. Educates patients on all aspects of self care and prescribed medical regimens for treatment of diabetes. 2. Assess inpatient needs for diabetic education. 3. Provide follow up care to all patients enrolled in the diabetes education program. Documents diabetes education and follow up. 4. Cooperate with public relations department to provide community service and to educate the public about diabetic issues. 5. Maintain clinical expertise and professional development in areas related to diabetic education. 6. Contribute to the development of written policy and procedure. 7. Works with laboratory to monitor use of blood glucose meters on patient units. Provide in-service education and other corrective action as indicated. 8. Participate in the development and management of the diabetes program budget. 9. Acts as a liaison among the program staff, advisory committee, and the administration of the institution. 10. Provides and/or coordinates the orientation and continuing education of the professional program staff. 11. Participates in the planning and review of the program each year. 12. Evaluates program effectiveness. 13. Serves as a member of the advisory committee. 14. Over sees the program with on-site supervision. 15. Adheres to medical center policies. 16. Maintains patient confidentiality and treats patients and families with dignity. 17. Demonstrates knowledge of and compliance with the medical center code of conduct and applicable corporate policies and procedures. 18. Performs other related duties as assigned or requested.
Required:
Associate's Degree in Nursing is required. A minimum of two years experience is required. KY Registered Nurse Licensure is required. Eligibility for Diabetic Educator Certification is required. If not certified upon hire, you must obtain certification within three years of hire.
Preferred:
Bachelor's Degree in Nursing is preferred Diabetic Educator Certification is preferred. To provide education to diabetic patients within the standards of the American Diabetes Association.
