What You'll Do Long-Term Care Education
- Develop and implement an annual education strategy for long-term care partners.
- Provide ongoing education to nursing homes, assisted living communities, hospitals, physician practices, and community organizations.
- Educate facility staff regarding hospice eligibility, symptom management, advance care planning, goals-of-care conversations, dementia care, communication, grief support, and end-of-life best practices.
- Maintain records of all educational activities and continuing education provided. Clinical Staff Development
- Educate and coach WV Caring nurses, aides, social workers, chaplains, volunteers, and interdisciplinary team members caring for patients in long-term care settings.
- Participate in orientation and onboarding of new clinical staff.
- Reinforce Medicare Conditions of Participation, documentation standards, regulatory requirements, and organizational expectations.
- Assist with annual competency validation, Perfect Visit education, and continuing professional development. Clinical Quality & Regulatory Excellence
- Promote evidence-based hospice practice throughout long-term care settings.
- Identify educational opportunities through quality audits, surveys, chart reviews, complaint trends, and performance metrics.
- Develop corrective education plans when deficiencies are identified.
- Participate in CHAP accreditation preparation (12/31:2028), state survey readiness, and quality improvement initiatives. Strategic Facility Partnerships
- Serve as WV Caring's primary educational liaison to long-term care facilities.
- Build collaborative relationships with Administrators, Directors of Nursing, Medical Directors, Social Workers, Case Managers, and facility leadership.
- Participate in facility Quality Assurance and Performance Improvement (QAPI) meetings as appropriate.
- Support referral growth through education, consultation, and relationship development.
- Collaborate with Marketing and Business Development on strategic facility outreach initiatives. Leadership
- Collaborate with the Director of Clinical Education & Professional Development on organizational education initiatives.
- Participate in interdisciplinary team meetings, leadership meetings, and special projects.
- Mentor clinical staff and serve as a resource for hospice best practices.
- Promote a culture of clinical excellence, accountability, and continuous learning.
