Job Description

Apply Admissions Discharge Coordinator Washington, PA Apply Job Type Full-time Description Gateway Rehab Center (GRC) has a wonderful opportunity for a full-time Admissions Discharge Coordinator at our Lighthouse for Men program in Washington, PA. Our Coordinator is responsible for coordinating the intake and discharge processes of our halfway house clients. This position receives infrequent supervision and instruction but has regular interaction with direct and indirect managers. Responsibilities Takes referrals and completes intake paperwork. Conducts interviews of patient referrals for admission to the program to assess patient appropriateness. Completes admission assessments for new patients as needed. Presents admission information to Treatment Team for their approval. Maintains contact with waiting list clients/referral sources. Updates assessments and completes admission paperwork when patient arrives. Schedules admissions, maintains updated bed assignment/waiting list. Sends daily census report as directed. Participates in Clinical Team meetings. Reports day to day progress of social service activities to clinical team in a timely and accurate manner. Ensures transportation for scheduled admissions. Maintains accurate documentation of patient contacts and discharge disposition planning. Completes referrals to housing programs per therapist's recommendation. Arranges aftercare appointments for both CD and MH providers. Coordinates patient services with outside agencies when needed (domestic violence, trauma, etc.) Assists patients in securing safe housing by arranging halfway or ¾ house placement interviews. Assists patient in completing Discharge Planning Guide and Continuing Care Plan. Facilitates Continuing Care group and other social services groups as assigned by supervisor. Coordinates patient in/out flow. All other duties as assigned by the Program Supervisor or Administrator. Attends mandatory Gateway training and in-services. Knowledge, Skills, and Abilities Familiarity with electronic health records (EHR) and other administrative tools. Understanding of ethical issues related to treatment, consent, and client rights. Understands the confidentiality laws, such as HIPPAA, and the importance of maintaining client privacy. Ability to develop and maintain relationships with referral sources and follow-up services. Requirements High school diploma or GED with some experience in a human service setting. Additional requirements Pass PA Criminal Background Check. Obtain PA Child Abuse and FBI Fingerprinting clearances. Pass drug screen. 2-Step TB Test Work Conditions Substance Use Disorder Treatment Center Favorable working conditions. Minimal physical demands. Significant mental demands include routine decision making, continuous attention to detail and high mental and visual concentration.
