PACE, a member of Empath Health is seeking a compassionate CNA/Program Aide to join our interdisciplinary PACE team in Pinellas Park, FL. Under the direction and supervision of the Day Center Supervisor, the CNA/Program Aide is responsible for providing and assisting participants with activities of daily living, restorative and supportive care. Team-Driven, Preventive Focus:

Collaborate with physicians, nurses, social workers, and other clinicians in a true interdisciplinary care model focused on prevention and holistic well-being. What You'll Do Provide personal care, including bathing, grooming, skin care, and oral hygiene. Assist participants with mobility, transfers, ambulation, and prescribed exercises. Prepare meals, assist with feeding, and support therapeutic diets. Help participants with toileting needs, including use of commodes, bedpans, and urinals. Assist with self-administered medications as directed. Monitor and report changes in participant condition to the Registered Nurse. Perform basic health-related tasks, such as taking vital signs and providing non-sterile care as instructed. Complete required documentation and maintain participant confidentiality.
