Job Description

Job Summary :
The Residential Program Assistant is responsible for assisting the Family Home Provider (FHP) program in Louisville and will provide direct support for the Louisville Residential Program Coordinator. Act as back up for Residential Program Coordinator by assisting with communication, recruiting, and attendance at Participant team meetings or meet & greets, as needed Coordinate new Family Home Provider (FHP) training and/or retraining - day to day expectations, documentation, responsibilities, etc. Complete monthly home visits Attend weekly Residential Team meetings to review referrals and any ongoing needs of the program or Participants Act as liaison with Participants, families and/or guardians to perform service quality check-ins, sharing concerns with Program Coordinators for any follow up Review and track monthly documentation (i.e. summaries, evacuation drills, daily logs, home visits and respite balances) Review and update Participant information in Oasis, including goals and objectives, to ensure current resources are available to staff providing services Communicate with Quality Assurance Specialist regarding Participant updates in Oasis Confirm with HR that staff have completed all necessary tasks prior to beginning work Assist with completion of participant forms (i.e. consents, releases, abuse & neglect), as needed Other duties as assigned
Education/Training:
At least a high school diploma or GED is required. Proficiency in MS Office and at least three (3) years administrative experience preferred. Previous experience working with Medicaid Waivers and/or individuals with disabilities a plus.
Qualifications:
Must be at least 18 years of age. Strong verbal and written communication skills are required. Attention to detail, accurate data entry experience, general math knowledge, and solid organizational skills are important. Positive attitude and ability to work independently or in a team environment are essential. Reliable transportation, valid driver's license and current car insurance are required.
Physical Demands:
Individual must be able to sit for long periods, bend, reach, and lift up to 20 pounds. This position is in an office environment. Path Forward of Kentucky provides supported employment, job coaching, respite, Family Home Providers, and community living supports for children and/or adults with disabilities. $16.00 per hour Expected hours: 35.0 - 40.0 per week
