COUNTRY CLUB DAY ACTIVITY PARAPROFESSIONAL JOB DESCRIPTION

Position Title:

Paraprofessional Position Description:

The Paraprofessional, under the supervision of a Qualified Professional, provides skill building training to individuals with mental illness, including those with the dual diagnosis of developmental disability and mental illness or mental illness and substance abuse. The Paraprofessional provides input about client needs, participates in the person-centered treatment planning process and delivers direct care services to clients. The Paraprofessional is responsible for maintaining complete and accurate records of services delivered. The Paraprofessional must be willing to transport clientele to and from the PSR program in a fifteen-passenger van.

Qualifications and Certifications:

The Paraprofessional must meet one of the following requirements under 10A

NCAC 27G .0104

A Paraprofessional must be at least 18 years of age and be able to read, write, understand and follow directions. Have a High School Diploma or GED (if employed before November 1, 2001, no high school diploma or GED is required). Paraprofessionals must demonstrate the knowledge, skills and abilities required by the population served. A Paraprofessional must have a Supervision Plan that is updated annually. The plan must include the name of the employee, the name of the supervisor and the number of hours of monthly supervision. Supervision must be provided by a Qualified Professional who has experience with the population served.

Knowledge and Skills:

Successful performance as a Paraprofessional requires the following knowledge and skills:

• Ability to establish and maintain effective, supportive and therapeutic relationships with adults and/or children with disabilities and their families/caregivers.
• Ability to apply strategies and interventions related to individual goals as outlined in the person-centered plan.
• Ability to apply negotiation, mediation and advocacy skills on behalf of clients.
• Ability to communicate effectively orally and in writing.
• Ability to exercise sound judgment in analyzing needs and situations and taking appropriate response.
• Ability to accurately gather, evaluate, organize and document information about client treatment and service delivery.
• Knowledge of disabilities and disability diagnoses.
• Knowledge of disability and other community resources.
• Knowledge of the application of evidenced-based/ best practices in service delivery.
• Knowledge of cultural diversity and ability to apply cultural awareness to professional relationships.
• Knowledge of Federal, State and local laws, regulations, policies and requirements regarding the provision of mental health services including a knowledge of client rights, HIPAA and state confidentiality, service record management and documentation.
• Proficiency with word-processing, electronic forms and email personal computer applications may be required.

Duties and Responsibilities:

Specific duties as a Paraprofessional are based on the services outlined in the client's person-centered plan and may include:

• Assist clients to increase their functioning so that they can be successful and satisfied in the environments of their choice with the least amount of ongoing professional intervention.

Focus on skill and resource development related to life in the community and to increasing the participant's ability to live as independently as possible, to manage their illness and their lives with as little professional intervention as possible, and to participate in community opportunities related to functional, social, educational and vocational goals.

• Use the principles of recovery in the delivery of services, including equipping consumers with skills, emphasizing self-determination, using natural and community supports, providing individualized intervention, emphasizing employment, emphasizing the "here and now", providing early intervention, providing a caring environment, practicing dignity and respect, promoting consumer choice and involvement in the process, emphasizing functioning and support in real world environments, and allowing time for interventions to have an effect over the long term.

• Provide a supportive, therapeutic relationship between the providers, client, and family which addresses and/or implements individualized interventions outlined in the Person-Centered Plan in any of the following skills development, educational, and pre-vocational activities: o A.

community living, such as housekeeping, shopping, cooking, use of transportation facilities, money management; o B. personal care such as health care, medication self-management, grooming; o C. social relationships; o D. use of leisure time o E. educational activities which include assisting the client in securing needed education services such as adult basic education and special interest courses; and o F. prevocational activities which focus on the development of positive work habits and participation in activities that would increase the participant's

• Comply with all requirements of regulatory entities, policies and procedures of Country Club Day Activity and CARF standards.
• Provide input to the treatment team about the client's progress toward goals and treatment outcomes including recommendations for revisions to the plan.
• Provide timely documentation of all services delivered using State and agency approved policy and procedures.
• Maintain the integrity of all client documentation and client records in accordance with State, Federal HIPAA and agency policy.
• Protect the rights of all clients per DHHS and agency policy.
• Identify, report and document all incidents involving the client based on State, LME and agency policy and procedures. Reporting may include the reporting of abuse, neglect and exploitation to DSS according to State law.
• Participate in required staff meetings and client treatment planning meetings.
• Participate in ongoing staff development training to maintain professional and technical knowledge and skills and fulfill State and agency training requirements.
• Submit accurate, timely and complete records of time worked and expenses incurred.

Working Conditions:

The Paraprofessional spends most of the service delivery time working face to face with the client. The Paraprofessional responsibilities may involve transporting the client for services directly related to the PCP. The Paraprofessional must always have immediate access to a copy of the client's PCP and Crisis Plan.
