Job Description

Job Overview We are seeking a dynamic and experienced Director of Care Coordination to lead our clinical care management team with passion and precision. In this vital role, you will oversee the development and implementation of comprehensive care plans, ensuring seamless coordination across various healthcare services. Your leadership will drive excellence in patient care, compliance with healthcare regulations, and the achievement of organizational goals. This position offers an exciting opportunity to influence healthcare delivery at a strategic level while fostering a collaborative, patient-centered environment. Responsibilities Lead and manage a multidisciplinary clinical team, including nurses, social workers, and case managers, to deliver exceptional patient care. Develop, monitor, and optimize care coordination processes aligned with NCQA standards and healthcare regulations such as HIPAA, Medicaid, and Medicare. Oversee discharge planning, medical records review, and documentation to ensure accuracy and compliance. Implement utilization review strategies to promote appropriate resource use across acute care settings. Coordinate with hospital departments, outpatient services, home health agencies, hospice providers, and behavioral health teams to facilitate smooth patient transitions. Utilize data collection and analysis to improve care plans, patient outcomes, and operational efficiency. Ensure team adherence to medical terminology standards and maintain up-to-date knowledge of health regulation policies. Requirements Extensive experience in hospital or acute care environments with a focus on trauma centers (Level I or Level II experience preferred). Proven leadership in clinical case management, nursing supervision, or healthcare management roles. Knowledge of managed care processes such as Medicaid and Medicare regulations; familiarity with NCQA standards is highly desirable. Experience in discharge planning, utilization review, and managing complex cases involving social work or addiction counseling. Demonstrated ability to lead teams effectively while fostering a collaborative environment focused on patient-centered care. Excellent understanding of medical terminology, anatomy, physiology, mental health counseling techniques, and behavioral therapy practices. Ability to navigate health regulation policies related to HIPAA compliance and documentation review processes. Skilled in data collection methods for quality improvement initiatives; proficient in managing medical records systems.
