26/hr Position Overview The Home Care Coordinator supports the coordination and implementation of home care services for PACE participants.
This role conducts home visits to assess home safety and care needs, coordinates with caregivers and vendors, assists with Durable Medical Equipment (DME) needs, and serves as a liaison between the participant, family, home care providers, and Interdisciplinary Team (IDT). Key Responsibilities Conduct home visits to assess participant safety, accessibility, ADLs, caregiver needs, and overall home care requirements. Coordinate home care services with vendors, family caregivers, and participants. Collaborate with the IDT to evaluate caregiver hours, DME, incontinence supplies, and other participant needs. Educate participants, families, caregivers, and vendors regarding approved services and care plans. Participate in IDT, Care Coordination, and staff meetings. Serve as a liaison between PACE and home care providers. Document assessments, services, and follow-up activities in accordance with departmental guidelines. Protect participant confidentiality and promote dignity, independence, and participant rights. Qualifications Graduate of an accredited Certified Nursing Assistant (CNA) program. Current California CNA certification required. Minimum of 1 year of experience as a CNA or healthcare worker; experience with frail or elderly populations strongly preferred. Current BLS certification required. Bilingual in English and Spanish, Mandarin, or Cantonese required, depending on location. Valid California Driver's License and reliable transportation with required auto insurance. Strong communication, organization, and interpersonal skills. Ability to work effectively with geriatric participants with complex medical and psychosocial needs. Ability to travel to participant homes and perform physical activities including standing, walking, bending, stooping, and lifting up to 15 lbs.
Pay:
$23.00
$26.00 per hour Expected hours: 24.0
