Job Summary Seeking an experienced Medical Social Worker (MSW) to provide inpatient acute-care social work services, including care coordination, patient assessment, discharge planning, and collaboration with interdisciplinary teams. The role supports patients across various age groups and requires strong knowledge of healthcare resources, insurance, community services, and discharge needs. Roles & Responsibilities Perform biopsychosocial assessments and initial evaluations. Assess patient and caregiver needs and develop appropriate plans of care. Provide care coordination and discharge planning for acute-care inpatients. Coordinate home care, DME, rehabilitation, and long-term care services. Collaborate with internal and external healthcare teams. Participate in care conferences and family meetings. Provide crisis intervention and support for patients and families. Assist with complex psychosocial, safety, and vulnerability concerns. Coordinate services related to HMOs, IPAs, insurance, and prior authorizations. Provide appropriate referrals to community resources and support programs. Document assessments, care coordination, patient responses, and discharge plans in Epic. Support patients with mental health concerns, homelessness, substance use, protective services, and other psychosocial needs. Requirements MSW License Certificate 2 years of acute medical hospital inpatient Social Work experience Experience performing Care Coordination and Discharge Planning Travel experience required ; first-time travelers may be considered Epic experience required Knowledge of protective services, including APS/CPS Knowledge of advance directives and
