Social Services Coordinator Santa Anita Assisted Living | Temple City, CA Full-Time | $25-$28 per hour Make a Difference for Residents and Families Every Day Santa Anita Assisted Living is seeking a compassionate, organized, and resourceful Social Services Coordinator to support our residents and their families. This position is ideal for someone with experience in senior living, assisted living, healthcare, social services, case management, or human services who enjoys helping older adults and families navigate resources, services, benefits, and changing needs. The Social Services Coordinator works closely with residents, families, community leadership, healthcare providers, and outside agencies to help connect residents with appropriate services and resources while supporting a positive resident experience. What You'll Do Build positive, professional relationships with residents and their families. Assist residents and families in identifying social, emotional, and practical needs. Connect residents with community resources, healthcare services, transportation, benefits, government programs, and other support services. Support new residents and families during the transition into assisted living. Coordinate referrals and follow up on services and resources. Maintain regular communication with residents, families, community leadership, healthcare providers, and outside agencies. Assist residents and families as needs or circumstances change. Participate in resident meetings and interdisciplinary team discussions as appropriate. Maintain accurate and timely documentation of resident services, referrals, resources, and follow-up. Develop relationships with local senior service organizations, healthcare providers, government programs, and community resources. Maintain an up-to-date directory of resources available to residents and families. Provide residents and families with information regarding available programs, benefits, and services. Assist with resident and family concerns and coordinate appropriate follow-up with community leadership. Maintain resident confidentiality and comply with applicable policies and regulatory requirements. Collaborate with Wellness, Engagement, Dining, Sales, and community leadership to support the overall resident experience. Perform other related duties as assigned. What We're Looking For Experience in social services, senior living, assisted living, healthcare, case management, resident services, or human services preferred. Experience working with older adults and families strongly preferred. Degree in Social Work, Human Services, Gerontology, Psychology, Sociology, or a related field preferred, or an equivalent combination of relevant education and experience. Knowledge of community resources and programs available to older adults is a plus. Strong interpersonal and communication skills. Excellent organizational, documentation, and follow-through skills. Strong attention to detail. Compassionate and professional approach when working with residents and families. Ability to maintain professional boundaries and confidentiality.
