This role ensures timely, accurate, and coordinated communication between the provider, patient and external health care facilities to support continuity of care and optimize patient outcomes. Minimum Education Requirement :

High school diploma or GED preferred. Bilingual (Spanish) preferred. M i n i m um E x p er i e n c e Re qui reme nt : Three to five (3-5) years in a healthcare setting is preferred.
