The position will work two days per week at Palm City Wellness and two days per week at Campus Recovery Center , collaborating closely with Admissions, Outreach, Clinical, Case Management, Medical, and Operations. The Client Care Coordinator helps ensure that clients move smoothly through the continuum of care and that admission, discharge, transfer, and aftercare logistics are completed efficiently and communicated to all appropriate parties. Admissions & Pre-Admission Coordination Coordinate incoming admissions with facility staff based on direction and information provided by the Admissions and Outreach teams . Assist with scheduling and coordinating client pre-assessments prior to admission. Facilitate communication between Admissions/Outreach and facility staff regarding prospective clients. Help ensure necessary pre-admission information is obtained and communicated to the appropriate clinical or medical staff for review. Follow Admissions/Outreach directives regarding planned admission dates, arrival arrangements, transportation, and other admission logistics. Coordinate client travel and transportation arrangements related to admission when requested. Help ensure the facility is prepared for scheduled client arrivals. Maintain communication with clients, families, referral sources, and internal staff regarding admission logistics as appropriate. Client Engagement, Retention & AMA Prevention Develop rapport with clients and serve as an additional resource throughout their treatment experience. Proactively identify clients who may be considering leaving treatment prematurely. Participate in AMA-blocking and retention efforts when clients express a desire to leave treatment. Meet with clients to identify concerns or barriers contributing to premature discharge. Coordinate with Clinical and Operations to determine whether reasonable concerns can be addressed to support continued treatment. Contact authorized family members or other supports when appropriate to assist with retention efforts. Immediately communicate significant AMA or retention concerns to appropriate facility leadership and clinical staff. Track patterns and reasons for premature discharge to assist leadership with improving client retention. Family Communication Establish appropriate communication with authorized family members/supports. Provide families with a consistent point of contact for general care coordination and transition-related needs. Assist families with understanding admission, discharge, transfer, and aftercare logistics. Encourage appropriate family participation in the client's continuing-care plan. Maintain all communication in accordance with releases of information, HIPAA, and applicable confidentiality requirements. Aftercare & Discharge Coordination Work closely with the treatment team to help implement each client's recommended discharge and continuing-care plan. Help establish aftercare arrangements prior to discharge , including appointments, program admissions, recovery housing, outpatient services, and other recommended supports. Coordinate step-downs between Palm City Wellness and Campus Recovery Center when directed and clinically appropriate. Assist with referrals to outside providers or treatment programs when clients require services outside the organization. Coordinate with Admissions and Outreach regarding recommended referral destinations and placement directives. Communicate with receiving programs and assist with completion of required pre-assessments, screening calls, records coordination, and admission logistics. Confirm acceptance and anticipated admission dates with receiving programs when applicable. Help ensure clients leave treatment with clear information regarding their next level of care. Travel & Transportation Coordination Coordinate client travel associated with admissions, transfers, step-downs, and planned discharges. Assist with arranging flights, ground transportation, facility transportation, or other approved travel arrangements. Communicate itineraries and transportation plans to clients, families, receiving programs, and appropriate facility staff. Help resolve logistical barriers that could interfere with a successful admission or transition. Coordinate travel arrangements in accordance with Admissions/Outreach directives and facility approval processes . Referral & Step-Down Support Work with Admissions, Outreach, and Clinical leadership to facilitate appropriate internal and external referrals. Assist clients with completing required screening or pre-assessment processes for receiving programs. Facilitate warm handoffs whenever possible. Maintain communication with referral partners during the transition process. Help prevent gaps in care between discharge and admission to the next provider. Follow established Admissions/Outreach direction regarding referral relationships and placement coordination. Alumni Engagement Maintain appropriate contact with successfully discharged clients. Conduct post-discharge check-ins and encourage continued engagement with recommended aftercare. Help develop alumni activities and engagement opportunities across both facilities. Connect former clients with appropriate recovery-oriented resources and alumni programming. Identify former clients who may benefit from additional support or reconnection with services and communicate those needs to the appropriate team. Cross-Site Schedule Palm City Wellness - 2 days per week Campus Recovery Center - 2 days per week The Client Care Coordinator will work across both facilities and assist with coordination between programs when clients transition from one level of care or location to another. Core Objectives The Client Care Coordinator will help: Increase client retention and successful treatment completion. Reduce avoidable AMA discharges. Improve communication with clients and families. Ensure admissions and pre-assessments are efficiently coordinated. Improve discharge and aftercare planning. Increase successful step-downs and warm handoffs. Coordinate reliable client travel and transportation. Reduce gaps between levels of care. Support Admissions and Outreach directives related to client placement and transitions. Strengthen alumni engagement and post-discharge connection. Improve the overall client and family experience across Palm City Wellness and Campus Recovery Center. Qualifications Experience in behavioral health, substance use treatment, admissions, outreach, case management, client services, or care coordination preferred. Strong interpersonal and client-engagement skills. Ability to effectively engage clients who are ambivalent about treatment or considering premature discharge. Excellent organizational, communication, and follow-through skills. Familiarity with behavioral health levels of care and treatment transitions preferred. Ability to coordinate multiple admissions, discharges, referrals, and transportation arrangements simultaneously. Ability to collaborate effectively with Admissions, Outreach, Clinical, Medical, Operations, and Case Management. Understanding of HIPAA, confidentiality, releases of information, and professional boundaries.
