Job Description

Senior Helpers of Elmwood Park is seeking a part-time Client Care Coordinator to help ensure consistent delivery of care for our clients and well organized scheduling and communication for our caregivers. This role focuses on scheduling, client visits, nonmedical care planning, and monitoring caregiver training. What you'll do: Build and maintain caregiver schedules, fill open shifts, and respond to call-offs and service changes. Communicate schedule updates promptly to clients, families, caregivers, and office staff. Visit clients to check on service quality, discuss changing needs, and follow up on concerns. Prepare and update nonmedical service plans and caregiver task instructions in accordance with agency procedures. Monitor caregiver training assignments and completion, follow up on overdue requirements, and maintain accurate training records. Coordinate new service starts, caregiver matches, and transitions after a client returns home from a hospital or rehabilitation stay. Document visits, schedule changes, service updates, and follow-up actions in WellSky/ClearCare. Escalate urgent coverage, safety, or service concerns to the appropriate manager. What we're looking for: Experience in home care scheduling, care coordination, client services, or a related field preferred. Strong organization and communication skills, with the ability to stay calm and prioritize when plans change. Comfort using scheduling software, electronic records, and Microsoft Office. Ability to travel to client homes in our service area. Familiarity with dementia care or Illinois home care programs is a plus.
Pay:
$21.00 - $22.00 per hour Expected hours: 25.0 - 30.0 per week
