Marshall RehabilitationNursing is hiring a Social Services Coordinator to support residents and families and assist with admissions and discharge planning.

Pay:

$18.00-$21.00 per hour

Responsibilities:

Assist with admissions and resident/family support Coordinate discharge planning and outside services Participate in care planning and interdisciplinary meetings Address resident and family concerns Maintain accurate documentation Collaborate with nursing, therapy, and other departments

Qualifications:

Social services experience in skilled nursing or long-term care preferred Knowledge of admissions and discharge planning Strong communication and organizational skills Ability to work effectively with residents, families, and an interdisciplinary team Bachelor's degree in Social Work or related field preferred
